Circumstances:
The airplane was engaged in a local training mission at Chakulia AFB with a crew of five on board. While circling in the vicinity of the airbase, it collided with a second USAAF North American B-25 Mitchell registered 41-12671 and carrying a crew of six. Following the collision, both airplanes dove into the ground and crashed near the airfield. All 11 crew members were killed.

Circumstances:
The airplane departed Peterson AFB (Colorado Springs) on a navigation training exercise. En route, it entered an uncontrolled descent and crashed near Peyton, some 30 km northeast of the airbase. The airplane was destroyed and all six crew members were killed.

Probable cause:
Investigation disclosed that excessive use of the rudder tab control caused these early models of the B-25 to snap-roll resulting in a spin. This problem had previously been reported by pilots who lived to tell about it but the shortage of B-25 aircraft at Peterson Field kept them in service. This accident resulted in a report to the Field Service Command at Wright Field that may have corrected a design flaw. Future lives may have been saved by this tragic event.

The airplane departed Port Moresby-14 Mile Drome (Schwimmer) on a flight to Lockhart River (Iron Range), carrying three passengers and four crew members. Shortly after takeoff, while in initial climb, the copilot mistakenly retracted the flaps instead of the landing gear, which caused the bomber to stop climbing and begin to settle. It lost height and crashed near the Laloki River. The airplane named 'Diana's Demon' was destroyed and all seven occupants were killed.
