Circumstances:
The airplane departed RAF Thurleigh to attack a train factory located in Lille. In unknown circumstances, it collided with a second USAAF B-17 registered 41-24498 that was also carrying 10 crew members on the same operation. Following the collision, both airplane dove into the ground and crashed in the city of Marcq-en-Baroel. Three crew members were killed and seven others became PoW.

Circumstances:
After a crew was dropped at Pyote AFB, Texas, the airplane was flying back to base in Casper with seven crew members on board. While cruising at an altitude of 9,000 feet over New Mexico, the airplane suffered an in flight fire then exploded and crashed 5 km east of Medio, some 56 km north of Tucumcari. The airplane was destroyed and all seven occupants were killed.

Probable cause:
Investigators speculated that fumes accumulated in the bomb bay from an extra fuel tank that was mounted for the long flight. The explosion may have been triggered by an electrical short, or even a radio transmission.

The airplane departed McDill AFB, Tampa, on a flight to Marianna, north Florida. In unknown circumstances, it went out of control and crashed in Blountstown, some 45 km south southeast of Marianna Airfield. All six crew members were killed.

The four engine airplane departed Holloman AFB on a training exercise with a crew of 10 on board. While flying at night at low altitude, the captain snap the automatic pilot system when the airplane nosed down and crashed in a huge explosion 5 km west of Alamogordo. The airplane disintegrated on impact and all 10 crew members were killed. It is understood that a recovery was impossible due to an insufficient distance with the ground.

While circling in the vicinity of the airbase, it collided with a second USAAF North American B-25 Mitchell registered 41-12436 and carrying a crew of five. Following the collision, both airplanes dove into the ground and crashed near the airfield. All 11 crew members were killed.
