The single engine airplane departed Bhubaneswar-Biju Patnaik Airport at 1220LT on a schedule service to Rourkela, carrying four passengers and two pilots. Following an uneventful flight at an altitude of 8,000 feet, the crew started the descent to Rourkela when he encountered an unexpected situation and declared an emergency at 1314LT. Five minutes later, the pilot attempted an emergency landing when the airplane crash landed in a pasture located 10 km south of Rourkela. All six occupants were injured.

The three engine airplane departed Ankara-Esenboğa at 2017LT on a charter flight to Tripoli. At 2031LT, while cruising at an altitude of 32,000 feet, the crew sent a PAN PAN PAN message, reporting a malfunction of the electric system. The crew was cleared to return to Ankara and initiated a turn to the left. Two minutes later, the crew declared an emergency. Radio contact between ATC and crew was difficult then the airplane entered an uncontrolled descent and crashed in an open field located near the village of Kesikkavak, west of Haymana. The airplane was destroyed and all eight occupants were killed, among them General Mohammed Ali Ahmed Al-Haddad, Chief of Staff of the Libyan Armed Forces. He was returning to Libya following a meeting with Yaşar Güler, Turkish Minister of National Defense.

The airplane departed Dubai-Al Maktoum International Airport on a cargo service to Hong Kong on behalf of Emirates Sky Cargo (flight EK9788) with four crew members on board. After landing on runway 07L at Hong Kong-Chek Lap Kok Airport, the crew initiated the braking procedure. While decelerating to a speed of about 100 knots, the airplane veered off runway to the left, struck a ground service vehicle located on the perimeter road, went through the perimeter fence and plunged into the sea at a speed of 60 knots, coming to rest broken in two. All four crew members were rescued while both employees in the ground vehicle were killed. Preliminary data suggest that the engine n°4 was still accelerating to 106% N1 forward thrust after all selected thrust reversers were stowed. Number 1, 2 and 3 engine thrust levers were closed with reverse thrust levers selected to maximum while the n°4 engine reverse thrust lever was fully forward.

After the airplane has been leased for the summer period for forest fire survey missions, the contract was ended early October and the airplane was ferried back from Verkhneusinskoye to Shushenskoye. The crew departed Verkhneusinskoye at 1605LT and maintained regular radio contacts with their base, confirming that weather conditions were good. Nevertheless, while at halfway-point, weather conditions deteriorated. While cruising in clouds, the airplane collided with tall pine trees and crashed in a dense wooded area located some 40 km southwest of Tanzybei, bursting into flames. The airplane was destroyed by impact forces and a post crash fire and both crew members were killed.
Probable cause:
The accident resulted from the PIC's loss of visual contact with ground references when entering non-VFR weather conditions, resulting in a collision with obstacles and the ground.
The accident was the result of a combination of the following factors:
- The crew's unfounded decision to conduct a VFR flight along a route through mountainous terrain, without analyzing all necessary meteorological information.
- Failure to abort the flight and to return to the departure landing site when weather conditions deteriorated below established limits.
- Flight below safe altitude in conditions of limited visibility.

The pilot, sole on board, departed Morozov at 0708LT to perform a crop spraying flight on behalf of the Rodnik Farm. About two minutes later, while cruising at low height, the undercarriage collided with a 3-meter high embankment. The airplane flipped over and crashed inverted in a drainage ditch. The wreckage was found about 4 km south of the village. The airplane was destroyed and the pilot was killed.
Probable cause:
The accident was due to a collision of the main landing gear wheels with the embankment of an irrigation canal, which led to the destruction of the landing gear structure, the subsequent collision of the aircraft with the water surface of the canal, and a nose-over.
The accident was the result of a combination of the following factors:
- Loss of the captain’s working capability in flight due to acute cardiovascular insufficiency that developed as a result of a recurrent myocardial infarction against the background of hypertension.
- Possible influence on the captain’s health of the presence of ethyl alcohol in his blood.
- Performing the flight with an incomplete crew.
- The death of the captain in the aviation accident may have been facilitated by the absence of seat belts on the crew seats.

The airplane departed Khabarovsk on a schedule service to Tynda (flight 2G/AGU2311) with an intermediate stop in Blagoveshchensk, carrying 42 passengers (including five children) and six crew members. It departed Ignatyevo Airport at 1121LT on the second leg to Tynda. After the crew started the descent to the destination airport, he was cleared for a NDB approach via LMM and a landing on runway 06. Weather at destination was marginal with the base of scattered clouds at 210 metres. After the altimeters were set, the crew continued the approach when the radio altimeter alarm sounded. Three seconds later, the airplane collided with trees and crashed in a wooded area located 14 southwest of the airport, bursting into flames. The airplane was destroyed and all 48 occupants were killed.
Probable cause:
The accident resulted from a discrepancy between the altitude reference level based on the QNH pressure actually set on the barometric altimeters and the reference level used by the crew for assigned flight altitudes (based on QFE). This led to the aircraft flying approximately 600 meters below the assigned altitude, colliding with treetops and the ground.
The accident was the result of a combination of the following factors:
- The crew’s actual lack of preparedness to operate to aerodromes that had transitioned to QNH, which led to errors in the use of aerodrome pressure values (QNH/QFE) at Tynda aerodrome.
- The controller transmitting the QNH value, expressed in millimeters of mercury, without a request from the crew.
- The crew’s failure to monitor the consistency between the barometric pressure settings used and the assigned altitude values.
- The inability of ATC to monitor the aircraft’s flight altitude instrumentally due to inoperative secondary radar equipment at Tynda aerodrome.
- The crew’s deactivation of the aural Ground Proximity Warning System (GPWS) alert.
Final Report:
