Circumstances:
The seaplane departed Suva Airport in the night on a routine flight to Auckland. In unknown circumstances, it crashed into the Pacific Ocean off Fiji Islands. Few days later, some debris were found on beaches near Lauthala Bay. No trace of the 15 occupants was ever found.

The airplane was on a final stage of a transatlantic flight from the US to England via South America and Africa. On the last leg from Port Lyautey (Kenitra) to RAF St Eval, the crew encountered poor weather conditions. Due to the lack of visibility, the crew failed to realize he was off course when the airplane collided with a crag located near RAF Saint Davids. All four crew members were killed.

Circumstances:
The crew was performing a cargo flight from Moscow-Chkalovski to Belomorsk with an intermediate stop in Vologda. About 35 minutes into the flight, while cruising at an altitude of 200-250 meters, the right engine lost power and stopped. Shortly later, the left engine also failed. The Captain attempted to make an emergency landing when the aircraft hit tree tops and crashed 1,5 km northwest of the village of Zezevitovo. Four crew members and two passengers were killed. The aircraft was destroyed.
Probable cause:
The flight engineer forgot to switch the fuel selector in time, causing the fuel pressure to drop and the subsequent failure of the right engine soon followed by the left one.

While on a convoy coverage mission, the seaplane suffered a loss of engine oil supply in flight, which caused an engine failure. The pilot made an emergency landing in a sandy area located east of Santo Amaro do Maranhão (2° 26' S 43° 8' W). All 10 crew members escaped with minor injuries and the airplane was damaged beyond repair.

Probable cause:
The immediate cause for the accident was due to starboard oil cap becoming open in flight, thus releasing oil tank supply. Due to an oil starvation, the right engine subsequently failed.
