Circumstances:
The airplane departed Morrison AFB, West Palm Beach, on a flight to Dakar with intermediate stops in Belém and Natal. While approaching Guinea on the leg from Natal to Dakar, the engine n°3 lost oil and its propeller could not be feathered. Because of the drag caused by a windmilling propeller, the fuel consumption was higher than expected and the airplane ran out of fuel. The airplane was ditched in the Atlantic Ocean some 40 km west of Boffa. All crew members were rescued and the airplane sank and was lost.
Probable cause:
Oil leak on the engine n°3 and its windmilling propeller that could not be feathered caused an excessive fuel consumption, thus the airplane ran out of fuel.

The crew departed Pueblo Airfield on a navigational training exercise. En route, he encountered technical issues with the hydraulic system and decided to return to base for an emergency landing. The airplane landed about ⅓ of the runway length and due to a lack of braking power, was unable to stop within the remaining distance. It overran and crashed into a ravine located 176 metres past the runway end. The pilot and the flight engineer were killed and six other crew members were injured.

The airplane departed Roswell on a night practice bombing mission to Big Spring and Amarillo with a crew of nine on board. While returning to base in the early morning and cruising at an altitude of 22,000 feet, the crew encountered severe icing conditions when both port engines failed. The airplane entered a spin and disintegrated at a height of about 1,500 feet before crashing in an isolated area located 13 km northwest of Roswell. Eight crew members were killed while the flight engineer was able to bail out and was found alive.
