After takeoff from the Thomson Bay in Rottnest Island, the single engine seaplane banked left, causing the left wing tip to struck the water surface. The airplane water looped and plunged into the water, floating in a vertical attitude. Three passengers were seriously injured while one was unhurt. Two others (one Swiss and one Danish citizen) as well as the pilot were killed.

The twin engine airplane departed Canela Airport runway 24 at 0912LT on a private flight to Jundiaí, carrying nine passengers and one pilot. Weather conditions were marginal with limited visibility due to rain falls and fog. One minute later, the airplane collided with the chimney of a private house, lost height and crashed on an inn located in the center of the city of Gramado, about 3 km west of Canela Airport. The airplane was totally destroyed as well as several buildings. All 10 occupants were killed and 17 people on the ground were injured, two seriously. The pilot and owner of the airplane, Luiz Claudio Salgueiro Galeazzi, was travelling with his wife, three daughters, sister, brother in law, mother in law and two nephews.

The airplane then descended into the ground and crashed on a vacant building, bursting into flames. The airplane was totally destroyed and both occupants were killed.

During the takeoff roll on runway 22L at Mesa-Falcon Field Airport, after completing a distance of about 3,400 feet and at a speed of 133 knots, the crew decided to abort the takeoff procedure and initiated an emergency braking maneuver. Unable to stop within the remaining distance, the airplane overran, went through the perimeter fence, hit a car moving around on North Greenfield Road and eventually came to rest against trees, bursting into flames. A passenger was rescued while four other occupants, two passengers and both pilots were killed. The car's driver was also killed. The accident occurred in good weather conditions.

The crew attempted an emergency landing when the airplane collided with trees and crashed in a dense wooded area located about 1,5 km from the airfield. A passenger was killed and four other occupants were injured. The airplane was en route to Zheleznogorsk on behalf of a Gold Mine Company.

While contacting soft ground, it lost its undercarriage and slid for few dozen metres before coming to rest nearby a wooded area. All 48 occupants evacuated safely. The airplane landed at 1019LT from Biak and was taking off for its last leg to Jayapura when the accident occurred.

After takeoff from runway 25 at Troutdale Airport, the twin engine airplane climbed to an altitude of 1,700 feet when the pilot was contacted by ATC about the fact that his altitude was well above the VFR altitude restrictions for the Gresham-Troutdale area. The pilot reported handling problem when the airplane entered an uncontrolled descent and crashed onto a house located in Fairview, about two km southwest from runway 07 threshold, bursting into flames. Both occupants as well as one people in the house were killed. The airplane was en route to Saint George, Utah.
