Circumstances:
The Twin Otter departed Iquitos Airport at 1137LT on a flight to Pucallpa with intermediate stops in Requena, Orellana, Pampa Hermosa and Contamana. While descending to Pampa Hermosa in marginal weather conditions, the aircraft crashed in a mountainous area located few km from the airport. Seven passengers were rescued while 13 other occupants were killed.

On final approach to Saposoa Airport, at a distance of approximately 3 km from the airfield, the pilot-in-command lost visual contact with the ground due to adverse weather conditions. Despite the circumstances, the crew decided to continue the approach when, at an altitude of 50 feet, the visual contact was reestablished with the runway that was to the right of the airplane. The pilot-in-command (a student pilot) decided to initiate a go around procedure but in the same time, the instructor pilot took over the controls, reduced the engine power, made a turn to the right and landed the airplane about half down the runway. After touchdown, the crew started the braking procedure, but on a wet runway and with an insufficient remaining distance, the airplane failed to stop in time, overran and crashed into the Saposoa River. The instructor pilot and five passengers were killed while 24 other occupants were injured, some seriously. The airplane was destroyed.
Probable cause:
According to the analysis of the facts and the manifestations of the student pilot and the observer pilot, it is concluded that the instructor pilot decided to continue with the landing, in spite of the fact that the weather conditions were not adequate, and losing visual contact with the ground in the final approach, making visual contact at a very low altitude (50 feet), forcing the airplane to land, ignoring the suggestion of the other pilots to make a go around procedure and landing on the second third of the runway, which was wet due to the effects of the rain and did not allow an effective braking, so the airplane went off the runway, falling and sinking into the river.
The main cause of the accident was a pilot's personal error for not complying with visual flight rules and procedures.
Contributing factors were:
- Poor weather conditions in the area of the Saposoa Airport,
- Lack of coordination on part of the crew who failed to make a prior comment before landing procedure.

The crew completed the approach and landing in a nose down attitude, causing the nose gear to land first. Upon impact, the nose gear collapsed and the fuselage broke in two. Both parts slid for few dozen metres on the runway and while the rear part remained relatively intact, the front part came to rest, bursting into flames. 42 people were evacuated, among them 24 were injured. The captain, trapped in the cockpit, was killed.

While descending to Yurimaguas-Moisés Benzaquen Rengifo Airport, the right engine caught fire and exploded. For unknown reasons, the crew was unable to extinguish the fire that spread to the right wing. The crew initiated an emergency descent and attempted to land near Pongo de Caynarachi, about 50 km south of Yurimaguas. Upon landing, the aircraft lost its undercarriage and slid for few dozen metres before coming to rest, bursting into flames. Both pilots escaped uninjured while the aircraft was destroyed by fire.
Probable cause:
The exact cause of the fire on the right engine could not be determined.

The wreckage was found in a jungle near Campanilla and both occupants were killed. Weather conditions were considered as good but it is possible that is deteriorated locally.

While parked at Bellavista-Huallaga Airport, the crew was stopped by seven local police officers who suspected narcotic traffic. In a certain confusion, the crew ignored the police instructions and elected to takeoff. Shortly after rotation, while climbing to a height of 20 meters, the aircraft was hit by several bullets shot by the policemen. The crew lost control of the airplane that crashed by the runway. All 15 occupants were killed. It was later determined that the captain was killed instantly while the copilot died one hour after the accident as five bullets were found in his body.
Probable cause:
Shot down by local police officers. At the time of the crash, all policemen were intoxicated and elected to stop the airplane as they suspected narcotic traffic.
