Circumstances:
The airplane was ferried from England to Canada to be transferred to the RCAF. On approach to Gander Airport, it crashed for unknown reasons in a pond located near Benton, about 15 km southeast of Gander Airport. All seven crew members were rescued while the aircraft was destroyed.

On final approach to Wright-Patterson AFB, the crew failed to realize his altitude was insufficient. The airplane hit a tree with one of its wings, stalled and crashed, bursting into flames. All three crew members were killed. They were completing a cargo flight on behalf of the USAAF.

On final approach to RAF Attlebridge, the right engine failed. The pilot tried to maintain sufficient speed but the airplane stalled and crashed near the airbase. All three crew members were killed.

Probable cause:
Failure of the right engine on final approach following a bird strike. It was determined that the bird collided with the engine and blocked the carburetor, causing the engine to stop.
