Few minutes after takeoff from Ukunda-Diani Airfield, the single engine airplane entered an uncontrolled descent and crashed in Kwale, some 17 km northwest of the departure airport. The airplane was destroyed by impact forces and a post crash fire and all 11 occupants were killed, among them 10 tourists (eight Hungarians and two Germans) en route to the Masai Mara National Park.

The airplane departed Khabarovsk on a schedule service to Tynda (flight 2G/AGU2311) with an intermediate stop in Blagoveshchensk, carrying 42 passengers (including five children) and six passengers. It departed Ignatyevo Airport at 1121LT on the second leg to Tynda. While descending to Tynda Airport, it appears that the crew encountered marginal weather conditions with reduced visibility due to low ceiling. On approach, the airplane impacted trees and crashed in a wooded area located about 15 km from the airport. The airplane was destroyed and all 48 occupants were killed. Apparently, the crew was attempting a second approach when the accident occurred. At the time of the accident, weather conditions were as follow: light rain, few CB's at 210 metres and cloudy at 600 metres.

The airplane, a Boeing 787-8 Dreamliner, flight AI171, departed Ahmedabad-Sardar Vallabhbhai Patel International Airport on a regular schedule service to London-Gatwick, carrying 230 passengers and 12 crew members. After takeoff from runway 23, the airplane climbed to an altitude of 625 feet with the undercarriage still down. Then it started to descend, first in a relative flat attitude, then nosed up (increasing the angle of attack) until it collided with several residential buildings located near Ghoda Camp Road, some 1,500 metres from the runway end, bursting into flames. The airplane was totally destroyed by impact forces and a post crash fire. Weather conditions at the time of the accident were considered as good. It appears that the crew issued a mayday call shortly after liftoff. Few hours after the crash, local authorities confirmed that one passenger survived while 241 other occupants were killed as well as 19 people on the ground. 67 other people on the ground were injured. Runway 23 is 3'600 metres long and it seems that the rotation was completed very late. An analysis of the Airport CCTV shows that the airplane ceased to climb about 12 seconds after rotation, and that the final impact occurred some 30 seconds after liftoff. It appears that the RAM Air Turbine (RAT) was deployed at the time of the accident, suggesting issues with engines.
Probable cause:
In the below preliminary report, its is reported that the aircraft achieved the maximum recorded airspeed of 180 Knots IAS at about 08:08:42 UTC and immediately thereafter, the Engine 1 and Engine 2 fuel cutoff switches transitioned from RUN to CUTOFF position one after another with a time gap of 01 sec. The Engine N1 and N2 began to decrease from their take-off values as the fuel supply to the engines was cut off. In the cockpit voice recording, one of the pilots is heard asking the other why did he cutoff. The other pilot responded that he did not do so.

The airplane departed Panama City-Marcos A. Gelabert Airport at 2013LT on a regular schedule service (flight PST982) to Bocas del Toro, carrying 35 passengers and three crew members. The approach to Bocas del Toro-Capitán José Ezequiel Hall Airport was completed at night and the runway was wet by recent rain falls. After touchdown on runway 27, the airplane started to slid then veered off to the right. While contacting soft ground, the undercarriage were torn off and the airplane slid for few dozen metres. The right wing was sheared off while contacting a fence then the airplane came to a halt with the empennage that also separated from the main fuselage. All 38 occupants were quickly rescued and there were no serious injuries.

At this time, the airplane was about 70 km from the destination airport. The wreckage was found a day later on sea ice in the Norton Sound approximately 34 miles (54,7 km) southeast of Nome. Following an uneventful flight, the crew was cleared to start the descent from the south. On final approach to runway 33, after being cleared to land, at an estimated altitude of 350 feet, while approximately one km short of runway, the airplane collided with a US Army Sikorsky UH-60 Blackhawk registered 00-26860 and carrying three crew members. Under callsign PAT25, the helicopter was approaching Joint Base Anacostia-Bolling when the collision occurred. Both airplanes dove into the Potomac River. All 64 occupants were killed, among them two world-champion Russian figure skaters as well as 'several members' of the US figure skating community. All three occupants in the Black Hawk were also killed. First fatal accident involving a CRJ-700 and worst accident in the US since November 2001.
