Circumstances:
The twin engine airplane departed Ciudad del Este-Guaraní Airport Runway 05 at 1523LT on a cargo flight to Asunción. On board were two pilots and two security officers from Prosegur in charge of the cargo consisting with valuable objects. About 12 minutes into the flight, while cruising at an altitude of 6,400 feet and at a speed of 150 knots, the crew encountered engine problems and decided to return to Ciudad del Este. On approach from the west, the airplane lost height and crashed against trees located near the village of San Isidro, some 1,500 metres west of the main terminal. The pilot was killed and three other occupants were seriously injured. The airplane was destroyed.

Circumstances:
The airplane departed La Paz-El Alto at 0819LT on a positioning flight to Santa Cruz-El Trompillo Airport with two pilots on board. The last contact with ATC was recorded at 0850LT. About five minutes later, while cruising at FL390, the airplane initiated a turn to the right then completed a 360° circuit that was repeated 20 times until 1055LT. At this time, it entered an uncontrolled descent (the last radar contact was recorded at 19,000 feet) and eventually crashed in a flat attitude in a wooded area located northeast of San Gabriel. Both crew members were killed.

Probable cause:
A possible slow and accidental cabin depressurization which caused the pilots to suffer hypoxia and resulting in a loss of control after the airplane ran out of fuel.

About 20 minutes into the fight, while cruising at an altitude of 6,500 feet in poor weather conditions, the airplane contacted trees and crashed in a dense wooded area located on the slope of a mountain in the region of Cuyuni-Mazaruni. The pilot was killed.

The airplane departed Puerto Leguízamo-Caucayá (La Tagua) Airport on a troop transfer to Puerto Asís, carrying 117 soldiers from three platoons and 11 crew members. Shortly after takeoff from runway 10, while in initial climb, it encountered engine problems, rolled slightly to the left then descended to the ground and crashed in a wooded area located 3 km northwest of the city, bursting into flames. 57 people were rescued and 69 others were killed, 61 soldiers, six crew members and two police officers.

The four engine airplane departed Santa Cruz-Viru Viru Airport on a flight to La Paz, carrying eight crew members and a load of new banknotes. The approach to La Paz-El Alto was initiated in marginal weather conditions and after landing on runway 28, the airplane was unable to stop within the remaining distance, overran and collided with several vehicles on the Costanera Avenue. The airplane was destroyed and among its occupant, one was killed while seven others were injured. On the ground, several cars and trucks were destroyed, 23 people were killed and 37 others were injured, some seriously.

Seven minutes later, it reached the maximum altitude of 12,900 feet then the crew started the descent to Ocaña Airport. While descending at an altitude of 7,900 feet in limited visibility due to patches of fog, the airplane impacted the slope of a mountain located near the village of Curasica. All 15 occupants were killed.

The crew encountered unknown technical issues and were forced to attempt an emergency landing. The twin engine airplane crash landed on an embankment along road BR-364 in Extrema, west Rondônia. All occupants evacuated safely, among them Federal Deputy Maurício Carvalho and City Councilor Márcio Pacele who were travelling with advisors from Porto Velho.
