The airplane was engaged in an operation to Velikiye Luki when the left engine caught fire. The pilot ordered his crew to bail out and elected to return to base when he lost control of the airplane that crashed in Grylevo. Three crew members parachuted to safety while the pilot was killed. The wreckage was found in a dense wooded area in October 2019.

Circumstances:
The pilot departed Burluk on a communication flight to Olkhovka. En route, weather conditions worsened and his decision to return to Burluk was taken too late. In heavy snow falls, he lost control of the airplane that dove into the ground and crashed in Ostrovskaya. The pilot was killed. At the time of the accident, the visibility was less than 15 metres with strong winds and heavy snow falls.
Probable cause:
The accident was the consequence of the combination of the following factors:
- Deterioration of the weather conditions in flight with heavy snow falls and low visibility.
- The late decision of the pilot to return to his departure point after the weather deteriorated.
- The Squadron Leader cleared the pilot to takeoff based on the actual weather conditions at the departure and arrival points, without analyzing the situation as a whole.

Because the Squadron Leader was not present that day, the pilot and a flight engineer departed Berezhnovka at 0830LT without permission on a flight to Kamychyn. In addition to the crew were four passengers on board: the Kamychyn President' spouse and her three children. After landing in Kamychyn, the crew was invited by the passengers to spend time in their house and they drank one liter of vodka in two hours. At 1400LT, they departed Kamychyn for a return trip to Berezhnovka. On final approach, at a height of about 200-300 metres, the pilot made successive turns and while completing a left tun to an angle of 50-60°, the left wing broke off. The airplane dove into the ground and crashed. Both crew members were killed.
Probable cause:
The cause of the crash was the destruction of the aircraft in flight due to:
- The overloading of airplane parts during the steep turn and recovery,
- The presence of an old crack in the cross-section of the fork bolt securing the left front strut up to 60% of the surface,
- The flight was carried out by an intoxicated pilot.

While approaching Saratov, the crew encountered foggy conditions. In poor visibility, the airplane impacted the slope of a mountain and crashed. The airplane was destroyed and both crew members escaped unhurt.
Probable cause:
The accident was caused by the combination of the following factors:
- The flight was conducted below minimum weather conditions,
- The flight was conducted with a non operational airspeed indicator,
- Careless determination of actual flight altitude,
- The pilot made a turn without gaining altitude.

The airplane departed Seymchan on a flight to Yakutsk with a crew of three on board. It was part of a group of three Havoc transferred to Yakutsk. En route, weather conditions deteriorated with icing. A first airplane diverted to Oymyakon, a second landed safely in Yakutsk and 41-3319 crashed in a mountains area near Arkagala. The pilot and the radio operator were killed in the airplane and the dead body of the navigator was later found few km from the crash site after he bailed out.
Probable cause:
It is believed that the pilot attempted an emergency landing in hilly terrain while flying in poor weather conditions with a limited visibility due to snow falls. While cruising in icing conditions, the crew failed to activate the deicing equipment and did not activate the heating system for the Pitot tube and the carburetor. The failure of the speed indicator, engine vibrations and airframe icing forced the pilot to attempt an emergency landing.

Shortly after takeoff from Krasnoyarsk-Yemilianovo Airport, while in initial climb, the airplane stalled and crashed on a huge explosion on a parked II-4 (c/n 7116). Both airplanes were destroyed and all 30 occupants in the PS-84 were killed. On board were 10 civilians and all passengers were Soviet pilots and engineers flying to Ladd Army Air Field near Fairbanks to take delivery of several American bomber airplanes that should be delivered to the Soviet Air Force.
Probable cause:
The loss of control after takeoff was caused by the combination of an excessive accumulation of ice on airframe, wings and elevators and the fact that the airplane was operated beyond the MTOW.
