Circumstances:
While on a reconnaissance mission, the airplane was shot down by a German fighter and crashed near Demyansk. Three crew members were killed and one parachuted to safety. The wreckage was discovered in 2015.

The airplane crashed on takeoff from Migalovo AFB and was destroyed. All three crew members were killed.

Circumstances:
The airplane was engaged in an operation to Velikiye Luki when the left engine caught fire. The pilot ordered his crew to bail out and elected to return to base when he lost control of the airplane that crashed in Grylevo. Three crew members parachuted to safety while the pilot was killed. The wreckage was found in a dense wooded area in October 2019.

While on a test flight in the region of Moscow with three crew members on board, the airplane suffered an engine failure and crashed in Korolev. Two crew were killed and a third was injured.
