Few minutes after takeoff from Kyubyshev, the pilot encountered poor visibility due to fog. He decided to return to the airport when, from a height of 60-75 metres, he lost control of the airplane that crashed. The pilot was injured.
Probable cause:
The following factors were identified:
- Incorrect weather forecast issued by Kuibyshev AMSG,
- Violation of NLS by the pilot,
- Pilot negligence, demonstrated when changing the aircraft's course in fog, resulting in a loss of altitude.

Because the Squadron Leader was not present that day, the pilot and a flight engineer departed Berezhnovka at 0830LT without permission on a flight to Kamychyn. In addition to the crew were four passengers on board: the Kamychyn President' spouse and her three children. After landing in Kamychyn, the crew was invited by the passengers to spend time in their house and they drank one liter of vodka in two hours. At 1400LT, they departed Kamychyn for a return trip to Berezhnovka. On final approach, at a height of about 200-300 metres, the pilot made successive turns and while completing a left tun to an angle of 50-60°, the left wing broke off. The airplane dove into the ground and crashed. Both crew members were killed.
Probable cause:
The cause of the crash was the destruction of the aircraft in flight due to:
- The overloading of airplane parts during the steep turn and recovery,
- The presence of an old crack in the cross-section of the fork bolt securing the left front strut up to 60% of the surface,
- The flight was carried out by an intoxicated pilot.

The pilot departed Sukhumi on a supply mission to Pskhu. The airplane failed to arrive at destination and is believed to have been shot down by enemy fire and crashed near the Pereval Dou (Dou Pass) located about 11 km southeast of Pskhu Airfield. The pilot, sole on board, was killed.
Probable cause:
Probably shot down by enemy fire.

During the takeoff roll, somewhere in Russia, the airplane was unable to liftoff, ran off runway and came to rest. The pilot was injured and the airplane was damaged beyond repair.
Probable cause:
It was determined that the airplane contacted an obstacle during takeoff. The pilot failed to reject takeoff in time and reduced power too late, causing the airplane to overrun.

Without any permission, the pilot took off at 1542LT from Vyksa Airfield with two passengers on board. The purpose of the flight was to land at the new airfield under construction that should be deserving the Drugs Factory in Vyksa. The pilot completed two low approaches, one of them passing below high tension cables. While on a third approach, the airplane passed over houses then the pilot made a sharp turn when he lost control of the airplane that crashed onto houses, bursting into flames. All three occupants were killed and there were no casualties on the ground. Two hours, eight apartments and one grange were destroyed. The airplane was owned by Narkomzdrav, the People's Commission for Health.
Probable cause:
Lack of discipline on part of the pilot who initiated the flight without prior permission and lost control of the airplane after performing hazardous manoeuvres at low altitude over houses.
