The pilot was conducting a Part 135 on-demand cargo flight in instrument meteorological conditions. After executing a second missed approach, he informed air traffic control (ATC) of his intentions to divert to an airport located about 36 miles to the southeast. About 7 minutes after executing the second missed approach, the pilot began making unintelligible radio communications that ATC characterized as very garbled and difficult to understand, “almost hypoxic.” ATC then instructed the pilot to utilize oxygen. About 19 minutes after the second missed approach, ATC informed the pilot that the airplane had descended 1,600 ft, not following assigned course vectors or instructions and asked if everything was alright, to which no response was received. After attempting to relay communications through other airplanes in the area, an unintelligible response was received from the accident airplane. ATC then made numerous transmissions to the accident pilot urging him to utilize oxygen and open a window. No further communications were received from the accident airplane. A witness reported that he saw the airplane descend out of a low overcast cloud layer at a high rate of descent. The airplane then abruptly transitioned into a steep climb before re-entering the clouds. A few seconds later, he heard the airplane impact terrain and responded to the accident site. The sudden change in communications from the pilot indicates a possible impairment. When combined with the ATC data, the witness account, and the lack of any mechanical irregularities or malfunctions with the airplane, it is likely the pilot experienced difficulty controlling the airplane due to impairment. While the pilot was at an increased risk for an acute cardiovascular event, the extremely limited available medical evidence leaves no way to quantify that risk and no evidence regarding whether such an event occurred. As a result, whether the pilot was incapacitated by an acute medical event cannot be determined from the available medical information.
Probable cause:
An infight loss of control as a result of pilot impairment, the cause of which could not be determined.

The pilot reported that, upon reaching the decision altitude on a GPS instrument approach, he saw the runway end identifier lights and continued the approach. Shortly after, the lights disappeared and then reappeared. He continued the approach and landing thinking the airplane was lined up with the runway by using the runway edge lights for reference. Upon touching down about 225 ft left of the runway, the airplane dug into snow and flipped over, which resulted in substantial damage to the wings and tail. The pilot reported that there were no preaccident mechanical malfunctions or failures with the airplane that would have precluded normal operation.
Probable cause:
The pilot's improper decision to continue an instrument approach to landing following a loss of visual reference with the runway, which resulted in the airplane touching down left of the runway in snow and flipping over.

While the pilot was on a visual approach to the airport and descending over water on the left base leg, about 100 ft above the water's surface, the airplane entered instrument meteorological conditions with no forward visibility. The pilot looked outside his left window to gauge the airplane's altitude and saw "black waves of water approaching extremely rapidly." He tried to pull back on the yoke to initiate a climb, but the nosewheel contacted the water. Subsequently, the airplane nosed over and came to rest inverted in the water. The pilot reported that there were no preaccident mechanical malfunctions or failures with the airplane that would have precluded normal operation.
Probable cause:
The pilot's delayed response to initiate a go-around during a night visual approach over water after the airplane entered instrument meteorological conditions, which resulted in a loss of forward visibility and subsequent impact with the water.

The single engine airplane departed Hermosillo-General Ignacio Pesqueira García Airport at 0700LT on a schedule service (CFV872) to Guerrero Negro, Baja California del Sur, with one passenger and one pilot on board. About half an hour into the flight, radio and radar contact were lost with the airplane. Debris were found two days later in an uninhabited area located in the region of Tastiota, about 30 km southwest of Miguel Alemán. The aircraft was totally destroyed upon impact and both occupants were killed.

The airline transport pilot departed on a night cargo flight into conditions that included an overcast cloud ceiling and “hazy” visibility, as reported by another pilot. About one minute after takeoff, the pilot made a series of course changes and large altitude and airspeed deviations. Following several queries from the air traffic controller concerning the airplane’s erratic flight path, the pilot responded that he had “some instrument problems.” The pilot attempted to return to land at the departure airport, but the airplane impacted terrain after entering a near-vertical dive. The airplane was one of two in the operator’s fleet equipped with an inverter system that electrically powered the pilot’s (left side) flight instruments. Examination of the annunciator panel lighting filaments revealed that the inverter system was not powered when the airplane impacted the ground. Without electrical power from an inverter, the pilot’s side attitude indicator and horizontal situation indicator (HSI) would have been inoperative and warning flags would have been displayed over the respective instruments. The pilot had a history of poor procedural knowledge and weak flying skills. It is possible that he either failed to turn on an inverter during ground operations and did not respond to the accompanying warning flags, or he did not switch to the other inverter in the event that an inverter failed inflight. Due to impact damage, the operational status of the two inverters installed in the airplane could not be confirmed. However, the vacuum-powered flight instruments on the copilot’s (right side) were operational, and the pilot could have referenced these instruments to maintain orientation. Based on the available information, the pilot likely lost control of the airplane after experiencing spatial disorientation. The night marginal visual flight rules conditions and instrumentation problems would have been conducive to the development of spatial disorientation, and the airplane’s extensive fragmentation indicative of a high-energy impact was consistent with the known effects of spatial disorientation. Ethanol identified during toxicology testing may have come from postmortem production and based on the low levels recorded, was unlikely to have contributed to this accident. Morphine identified in the pilot’s liver could not be used to extrapolate to antemortem blood levels; therefore, whether or to what extent the pilot’s use of morphine contributed to the accident could not be determined.
Probable cause:
The pilot’s loss of control due to spatial disorientation. Contributing to the accident were the inoperative attitude indicator and horizontal situation indicator on the pilot’s side of the cockpit, and the pilot’s failure to reference the flight instruments that were operative.

The single engine airplane departed Manaus-Eduardo Gomes Airport Runway 29 in heavy rain falls as weather conditions deteriorated shortly prior to takeoff. After liftoff, while in initial climb, the airplane lost altitude and crashed in a dense wooded area located 600 metres past the runway end. The aircraft was destroyed by impact forces and all 10 occupants were injured, among them six seriously. At the time of the accident, weather conditions were poor with heavy rain falls, turbulence and windshear.
Probable cause:
The accident was the consequence of the combination of the following factors:
- Control skills – undetermined.
While facing adverse conditions, the use of controls may have been inappropriate for the situation and may have contributed to the aircraft not being able to maintain a positive climb rate.
- Attitude – undetermined.
Familiarization with the region may have led to an attitude, on the part of the PIC, of minimizing the importance of analyzing adverse weather conditions, to the detriment of compliance with the minimum limits established by the company in its manuals.
- Training – undetermined.
It is possible that, due to possible inadequate training, the SIC did not identify the critical situation that arose shortly after the take-off in time to assist the PIC in maintaining flight control.
- Tasks characteristics – undetermined.
The characteristics present in the type of operation, compliance with schedules without the possibility of delays, due to the runway closing period, may have favored the self imposed pressure on the part of the PIC, leading him to operate with reduced safety margins.
- Adverse meteorological conditions – a contributor.
The conditions at the time of the take-off contributed to the aircraft not being able to maintain the flight with a positive climb rate. The probable occurrence of Windshear determined that the trajectory of the aircraft was modified until its collision with the ground.
- Crew Resource Management – undetermined.
On the part of the SIC, no assertive attitude was perceived in the sense of alerting the PIC that those conditions were not favorable for takeoff. Thus, the crew decided to carry out the take-off despite the company's SOP.
- Organizational culture – undetermined.
The company encouraged compliance with the legs even though, within the planning of flights, there was not an adequate margin of time to absorb any delays. This culture may have influenced the PIC's decision-making, which, despite encountering adverse conditions, chose to take off, since the short time on the ground in the intermediate locations did not allow room for delays.
- Emotional state – undetermined.
The reports indicated that the PIC felt pressured to perform the take-off even in the weather conditions found on the day of this occurrence. Also, according to the interviewees, this pressure would be related to the fulfillment of the flight schedule and the need to keep to the scheduled times. In this way, it is possible that their assessment of the performance of the flight was influenced by the stress resulting from the pressure to complete the flight within the expected time, given the closing time of the runway for works.
- Flight planning – a contributor.
The flight planning was not carried out properly, considering that the planned schedules and routes would end after the closing time of the SBEG runway for works, provided for in the NOTAM. This meant that there was little time to adjust the legs, increasing the workload and stress in the cabin.
- Decision-making process – a contributor.
There was a wrong assessment of the meteorological conditions, which contributed to the decision of performing it in an adverse situation.

The TWR-EG did not inform, before the take-off, of the changes in the significant weather conditions that were occurring at the terminal, which could have contributed to the PIC's decision-making.

At 1101 Pacific Daylight Time on 06 August 2019, the Alkan Air Ltd. Cessna 208B Grand Caravan aircraft (registration C‐FSKF, serial number 208B0673) departed Rau Strip, Yukon, on a visual flight rules company flight itinerary to Mayo Airport, Yukon. The aircraft had 1 pilot, 1 passenger, and cargo on board. At 1113, the aircraft entered instrument meteorological conditions and struck rising terrain in a box canyon shortly after. The crash occurred approximately 25 nautical miles east‐northeast of Mayo Airport, at an elevation of 5500 feet above sea level. The Canadian Mission Control Centre did not receive a signal from the aircraft’s 406 MHz emergency locator transmitter. Eyewitnesses from a nearby exploration camp arrived at the site after approximately 1 hour. Royal Canadian Mounted Police and emergency medical services arrived on site approximately 90 minutes after the accident. The pilot and passenger received fatal injuries. The aircraft was destroyed; there was a brief post‐impact fire.

These are conditions, acts or safety deficiencies that were found to have caused or contributed to this occurrence.
1. The pilot’s decision making was influenced by several biases and, as a result, the flight departed and subsequently continued into poor weather conditions in mountainous terrain.
2. The high speed at low altitude and low forward visibility reduced the opportunities for the pilot to take alternative action to avoid terrain.
3. Within the box canyon, the canyon floor elevation increased abruptly within less than 1 NM and the low visibility prevented the pilot from detecting this and taking sufficient actions to prevent collision with terrain.
4. When the pilot turned into the box canyon, the terrain awareness and warning system aural alerts were ineffective in warning the pilot about the rising terrain either because he had already heard multiple similar alerts in the preceding minutes, or because he had silenced the alerts.

Findings as to risk:
