Circumstances:
In the evening of October 20, the four engine airplane departed Ayni AFB (Gissar AFB) located west of Dushanbe on a transport mission with a crew of eight on board. Shortly after takeoff, while climbing, the crew reported an engine fire and was cleared for an immediate return. The crew turned over the area for several minutes to dump the fuel before completing a landing on runway 08/26. After touchdown shortly past midnight on October 21, the airplane veered off runway and came to rest in a grassy area, bursting into flames. All eight crew members evacuated safely while the airplane was totally destroyed by fire. It is believed that the engine n°4 caught fire during initial climb.

The twin engine airplane departed Gonzales-Louisiana Regional Airport at 1456LT on a short flight to Patterson. Following a left hand turn, the airplane descended to Patterson-Harry P. Williams Airport when it crashed 300 feet short of runway 06 threshold, bursting into flames. The airplane was destroyed and both occupants were killed. Patterson-Harry P. Williams Airport is located 64 km southwest of Gonzales-Louisiana Regional Airport.

The airplane departed Chattanooga-Lovell Field Airport Runway 20 at 2225LT on a cargo flight (FDX1376) to Memphis with a crew of three on board. While climbing at night, the crew reported technical problems with the landing gear. The crew continued to the north, followed a circuit then completed a low pass over runway 20 at 2256LT. A second low pass was completed at 2319LT then the crew made a belly landing at 2347LT. The airplane slid on the runway then veered off runway and came to rest in a grassy area with both engines damaged. All three crew members evacuated safely.

After touchdown on runway 06L at Gao Airport, the airplane was unable to stop within the remaining distance. It overran and eventually crashed in a ravine located about 500 meters past the runway end, bursting into flames. There were some casualties as well as few people were rescued. The exact number of victims is unclear. It is believed that the airplane was given to the Malian Air Force by the Wagner Group about a week prior to the accident. Some members of the Wagner Group were on board according to the Malian Authorities. For unknow reasons, the airplane landed too far down the runway, most probably well past the mid runway, reducing the landing distance available. In such conditions, the crew was unable to stop the airplane in due time. It is believed that at least two crew members were killed in the crash.

On final approach to Barcelos Airport, the crew encountered poor weather conditions with reduced visibility due to heavy rain falls. On short final, the crew decided to initiate a go around procedure when the airplane apparently stalled and crashed on an embankment located near runway 09/27. The airplane was destroyed and all 14 occupants were killed. All 12 passengers were Brazilian tourists flying to Barcelos to practice sport fishing on the Río Negro. The airplane departed Manaus-Eduardo Gomes Airport on this charter flight approximately two hours prior to the accident.

Less than 50 minutes into the flight, he encountered an unexpected situation and was cleared to return to his departure point. On approach to runway 09L, he initiated a go around procedure then a turn to the left for a second circuit to land. While descending on final, he lost control of the airplane that crashed in a wooded area located in Elk Grove Village, less than 8 km west of O'Hare Airport. The pilot was slightly injured and the airplane was damaged beyond repair.

In unclear circumstances, the airplane caught fire in flight and the pilot diverted to Durant-Eaker Field. After landing on runway 17, the pilot stopped the airplane and was able to evacuate without injuries. The airplane was totally destroyed by fire.

On final approach to Kuala Lumpur-Subang-Sultan Abdul Aziz Shah Airport Runway 15, while completing a sharp turn to the right, the airplane entered an uncontrolled descent and crashed on a road located in Elmina, less than 5 km from the runway threshold, bursting into flames. The airplane was totally destroyed and all eight occupants were killed as well as two people on the ground, one motorcyclist and one car driver.
