The airplane departed RAF Hemswell at 1904LT on September 23 to lay mines in the French waters off Lorient. The airplane failed to return to base and is believed to have crashed somewhere into the Atlantic Ocean. Lost without trace.

In unknown circumstances, it collided with a second USAAF AT-11 Kansan registered 41-27620 also carrying a crew of four. Both airplanes dove into the ground and crashed some 15 km southeast of Florence. All eight crew members were killed.

Circumstances:
The crew departed Honiara-Henderson Airfield to attack Japanese ships cruising off Shortland and Bougainville Islands. While approaching the target area, the airplane named 'Bessie The Jap Basher' was shot down by the pilots of Japanese fighters, crashed into the sea and sank by a depth of 30 metres off Shortland Island. Eight crew members were killed while the captain was rescued and later executed by Japanese soldiers.

Circumstances:
The seaplane was en route from Lagos to Poole with intermediate stops in Bathurst and Lisbon. About 90 minutes after its departure from Bathurst, the aircraft crashed in unknown circumstances into the Atlantic Ocean off the Senegalese coast. The crew was unable to send any distress call. Few days after the accident, some debris and six dead bodies were found on the Senegalese coast and it is believed that the airplane apparently crashed some 50 km south of Dakar. All 19 occupants were killed.
Probable cause:
It is believed that the airplane suffered an explosion in midair, maybe following a sudden and uncontained failure on the fuel supply system.

Circumstances:
The airplane departed Dundalk-Logan Field, MD, to be delivered in Omaha, NB. When the pilots encountered rain and severe turbulence near Gary, IN, they realized that they could not continue and searched for a place to land. As they flew over McCool emergency landing field, Portage, IN, the pilot circled the airfield three times at low altitude. Unable to identify the boundaries of the airfield, the plane struck a utility pole, ripped through a fence and plowed a 800 metres swathe through a farmer's corn field before it came to rest. One engine had been torn from the nacelle, the other engine ripped from its mounts, one wing and both landing gears sheared off. The copilot had been thrown out of the airplane in his seat and landed 30 metres away from the wreckage.

Poor weather conditions.
