Probable cause:
Accident is attributed largely to state of weather and sea, poor visibility and glassy water. Contributing cause is believed to have been pilot fatigue.

While landing at night off Port of Spain, the seaplane crashed into the Gulf of Paria and sank. Ten occupants were killed and two others were rescued. The airplane sank and was lost.

The airplane departed Kodiak on a mission to attack Kiska. The crew encountered inclement weather near the target area, as well as heavy Japanese anti-aircraft fire. After the airplane entered a cloud bank, it impacted the slope of the Kiska Volcano and was destroyed upon impact. All seven crew members were killed.

The crew departed NAS Norfolk, Virginia, to ferry the airplane to NAS Natal, Brasil. En route from Belém and approaching the destination, the crew encountered very bad weather conditions with heavy rain falls. In zero visibility, while flying low, the airplane impacted the water surface and crashed into the sea some 8 km off Maxaranguape, north of Natal. Seven crew members were killed and three others were rescued 18 hours later.
