Circumstances:
The airplane departed Toluca-Licenciado Adolfo López Mateos International Airport on a positioning flight (Ambulance) to Cuernavaca, carrying two doctors and two pilots. After landing on runway 20 at Cuernavaca Airport, the crew initiated the braking procedure but the airplane was unable to stop within the remaining distance. It overran, went down a steep embankment and crashed in bushes, bursting into flames. The airplane was totally destroyed by impact forces and a post crash fire and all four occupants were killed. It is believed that the airplane landed too far down the runway, causing the landing distance to be insufficient.

Circumstances:
The airplane departed Veracruz-General Heriberto Jara Corona Airport Runway 01 at 2047LT on a private flight to Toluca, carrying one passenger and two pilots. Three minutes later, while climbing at night, it entered an uncontrolled descent until it crashed into the sea off Ciudad de Veracruz. The airplane was destroyed upon impact and all three occupants were killed.

Following an uneventful flight from Cancún, the crew encountered technical problems with the undercarriage while on approach to Toluca-Licenciado Adolfo López Mateos. Decision was taken to complete a belly landing on runway 15/33. The airplane slid for few dozen metres before coming to rest, bursting into flames. All 9 occupants evacuated safely and the aircraft was damaged beyond repair.

The crew continued the approach and landed on runway 15. The airplane fell on its belly, causing the right wing to struck the ground. The airplane slid for few hundred metres then veered off runway to the left and came to rest in a grassy area. All nine occupants were rescued and the aircraft was damaged beyond repair.

During the takeoff roll on runway 15 at Toluca-Licendiado Adolfo López Mateos, after the airplane passed the V1 speed, the warning light came on in the cockpit panel, informing the crew about the deployment of the reverse on the left engine. According to published procedures, the crew continued the takeoff and shortly after rotation, during initiale climb, the aircraft rolled to the left and crashed in an open field, bursting into flames. The wreckage was found about 200 metres past the runway end. The aircraft was destroyed and both pilots were killed.

Loss of control of the aircraft during the emergency procedure of indication of the deployment of the reverse of the engine number one during the takeoff roll above V1, which was consistent with the training and standard operating procedures that call to continue the takeoff, causing the collapse of the aircraft, by low speed and the operation of abrupt maneuvers and turns towards the engine side in idle position and commanded opening of the Drag Shut due to poor CRM.
Contributing factors:
- False indication of reverse display of dwelling position number one.
- Failure of pilots of previous flights not to refer failures to indicate the deployment of reverses for maintenance actions.

Following an uneventful positioning flight by night from Chetumal, the crew was cleared to land on runway 33 at Toluca Airport. Upon touchdown, the airplane went out of control and veered off runway to the right. While contacting soft grounf, the airplane lost its undercarriage and came to rest in a grassy area. All four occupants evacuated safely and the aircraft was damaged beyond repair.
Probable cause:
Loss of control upon landing due to the exhaust gases of the three turbofan engines and wake turbulences from an heavy category aircraft, causing a runway excursion, and an additional presence of crosswind, resulting in the aircraft rolling over uneven terrain of the safety strip resulting in severe damage.
Contributing factors:
- Safety strip in poor condition,
- Failure to attach times and separation distance between an aircraft taking off and landing on the same runway, to avoid the formation of wake turbulence and the presence of the exhaust gases of turbofan engines.

The crew departed Toluca Airport on a positioning flight to Zihuatanejo. Following an uneventful flight, the crew was cleared for a VOR approach to runway 26. Due to the formation of clouds in the vicinity of the airport, ATC changed the clearance and instructed the crew for a VOR/DME approach to runway 08. Following an unstabilized approach, the aircraft landed on a wet runway. After touchdown, the aircraft skidded and veered off runway to the left. In a grassy area, the left main gear impacted a concrete block hosting the electrical system for the runway and was torn off. Then the aircraft slid for few dozen metres before coming to rest. Both pilots evacuated safely and the aircraft was damaged beyond repair.
Probable cause:
Runway excursion due to loss of directional control of the aircraft during the landing run on an unstabilized approach.
The following contributing factors were identified:
a) Unstabilized approach,
b) Adverse atmospheric conditions in the "W" area of the airport,
c) Change of designation of runway in use for landing,
d) Lack of adherence to standard operating procedures "SOPS",
e) Lack of adherence to the concepts of "CRM" resource management in the cockpit,
f) Decreased situational awareness on the part of the commander of the aircraft,
g) Flying the approach and descent visually, following an IFR descent within IMC conditions (Instrument Meteorological Conditions),
h) Wet track,
i) Lack of crew coordination,
j) Poor judgement and incorrect decision,
k) Existence of a concrete marker with a level of 10cms protruding above the road surface in the runway safety zone.

The crew was apparently completing a positioning flight to Toluca Airport. Following a night landing on runway 15, the crew started the braking procedure when the aircraft deviated to the left. The crew applied full brake but the aircraft veered off runway. While contacting soft ground, the nose gear collapsed and the aircraft came to rest about 2,700 metres from the runway threshold. Both pilots escaped uninjured and the aircraft was damaged beyond repair.
