Circumstances:
En route from Irkutsk to Krasnoyarsk, the crew encountered poor weather conditions with rain and thunderstorm. Approaching the village of Petropavlovka, the airplane entered an uncontrolled descent and crashed in a marshy area. All three crew members were killed. The airplane was part of a group of six II-4 transferred to Krasnoyarsk. The wreckage was found in 2012.

Shortly after a night takeoff from Borki Airfield, the airplane failed to gain height, stalled and crashed. All 10 occupants were killed.

Probable cause:
The pilot flew during the preceding night and was tired at the time of the accident. Due to fatigue, he departed Borki Airfield without setting the elevator trim in the takeoff position, which prevented the airplane from gaining altitude after liftoff.

Circumstances:
The airplane departed Krasnoyarsk Airport at 0755LT on a flight to Novosibirsk with two crew members on board. It flew for about one hour and 10 minutes at an altitude of 400 metres and after all clouds disappeared, the pilot climbed to an altitude of 2,000 metres. At 1725LT, the airplane started to roll. The pilot ordered the mechanic to bail out and did the same shortly later. Out of control, the airplane dove into the ground and crashed some 10 km southeast of Anzhero-Sudzhensk. The pilot parachuted to safety but for unknown reasons, the mechanic remained on board and was killed.
Probable cause:
The cause of the accident was as follows:
- In-flight breakage of the right elevator trim rod, at the point where it is locked by a nut on the trim fork,
- Failure of the pilot to parry the pitch-up,
- When the airplane started to roll, the pilot was disoriented and missed the opportunity to take over control of the airplane. When the airplane started to loop, the increased load on the control column prevented him from controlling the airplane.

The airplane departed Chita on a delivery flight to Krasnoyarsk, part of a group of seven similar airplanes. On final approach, the pilot-in-command realized the airplane passed over the runway threshold by 50-70 metres and initiated a go around procedure. The airplane continued for about 150 metres at a height of 5 metres then climbed steeply to a height of 70-80 metres when it stalled and crashed, bursting into flames. Both crew members were killed.
Probable cause:
It was determined that the pilot failed to position the flaps in an appropriate angle for a go around procedure, causing the airplane to lose speed and height, to stall and to crash.

Shortly later, he lost control of the airplane that descended into the ground and crashed north of Nizhnie Tal'tsy. Both crew members were killed.
Probable cause:
Failure of the crew to follow the published procedure while continuing into bad weather conditions. Failure of the weather service to provide appropriate information, the bulletin transmitted to the crew was not relevant and did not reflect the actual weather conditions in the area.

The left engine lost pressure and power in flight, followed 18 minutes later by the right engine. The pilot reduced his altitude and made an emergency landing in a swampy area. Both crew members escaped uninjured while the airplane was destroyed by a post crash fire.
Probable cause:
Mismanagement of the fuel supply system on part of the crew.
