The airplane departed Sumburgh Airport on a maritime patrol flight off the Norwegian coast. While returning to base, it was shot down by a German fighter. The pilot attempted an emergency landing on Grunay Island when the airplane crashed and burnt. All three crew members were killed.

Circumstances:
The crew departed RAF Upwood on a low level training exercise, between 500 and 600 feet. Approaching the city of Birmingham in poor visibility, the airplane collided with the cable of a barrage balloon. Upon impact, the right wing was severed and the airplane entered a dive before crashing in Bearwood, in the suburb of Birmingham. All three crew members were killed.

The airplane was damaged beyond repair and both crew members were uninjured. The second airplane involved in the collision was repaired and returned to service.

The crew departed RAF Carew Cheriton on a convoy patrol mission. Returning to base, he encountered poor weather conditions over Pembrokeshire and got lost. Nevertheless, the pilot was able to locate the RAF Pembrey and attempted an emergency landing when the airplane crashed by the runway. The airplane was destroyed and all three crew members were injured. The wireless operator died two days later from injuries sustained.
