Circumstances:
The crew was performing a flight from Sterlitamak to Orsk and return via Zilair and Akyar, taking part to an oil pipeline survey mission on behalf of the oil company Transneft. While flying at low height in marginal weather conditions, the crew initiated a turn when the single engine impacted the ground and crashed in a snowy field located 2 km southwest of Gai, about 25 km north of Orsk. The aircraft was destroyed by impact forces and all three occupants were killed. There was no fire. At the time of the accident, the visibility was reduced to 800 metres and down to 300 metres locally. The cloud base was at 90 metres with freezing fog.
Probable cause:
Most probably the fatal accident with An-2 RA-40204 aircraft was caused by spatial disorientation by the crew turning in visibility restriction conditions (fog) and "whiteout" of snow underlying surface that resulted in uncontrolled descending and aircraft ground impact.
Most probably the following factors contributed to the accident:
- Incorrect evaluation of weather conditions by the crew resulted in unreasonable decision to perform VFR flight,
- Flight operation at the unauthorized law height above ground,
- Lack of IFR operation skills, training and checks for minima reduction by PIC were formal, weather conditions during operations didn't comply with assigned minima requirements.

Circumstances:
Following successful spraying missions, the crew who was intoxicated decided to perform a local 'private' flight and allowed five people to board the plane. Immediately after takeoff, while in initial climb, the aircraft stalled and crashed, bursting into flames. A pilot and four passengers were killed while two other occupants were injured. The aircraft was destroyed.
Probable cause:
It was determined that the aircraft stalled because its speed was too low after liftoff. Also, it was reported that all seven occupants were intoxicated.

Circumstances:
The single engine airplane departed Buguruslan Airport at 1332LT on a local training flight, carrying one pilot, one instructor and two cadets. Weather conditions were marginal due to snow falls. After several circuits were completed at an altitude of 700 meters, the crew informed ATC he was descending to 300 meters when control was lost. The airplane lost altitude and crashed in an open field located in Ponikla, bursting into flames. The aircraft was destroyed and all four occupants were killed.
Probable cause:
The exact cause of the accident could not be determined with certainty. However, it was reported that the engine failed en route after it got cold while operated in very low OAT. The lack of crew experience and skills in winter conditions was considered as a contributing factor.

Circumstances:
On final approach to Orenburg, the aircraft dropped below the glide path and the crew did not take the decision to attempt a go around. The three engine aircraft hit the ground 68 meters short of runway threshold with a positive aerodynamic force of 3 g, bounced and touched down again 635 meters further. On the second touchdown, the positive aerodynamic force was 3,9 g, causing the aircraft to brake in two. The airplane went out of control and eventually came to rest. Two crew members and one passenger were injured.
Probable cause:
Wrong approach configuration on part of the crew who failed to follow the approach checklist and published procedures. A series of errors and omissions on part of the flying crew led the aircraft descending below the glideslope and despite several alarms sounded, the captain failed to attempt a go-around manoeuvre. An inappropriate usage of approach charts, stabilizers at -5,5°, flaps at 45° and procedures violation caused the airplane to follow a wrong approach path. Also, the controller in charge of the approaches cleared the crew to land in lieu of giving the instruction to initiate a go-around.

Circumstances:
On a crop-spraying flight for the kolkhoz (collective farm) 'Pobeda' at Kamennaya Sarma (Buzuluk district of the Orenburg region), the engine failed (probably due to a carburetor problem) while the aircraft was flying at a height of some 20-25 metres near Kamennaya Sarma. As an immediate forced landing was not possible due to the terrain configuration, the pilot initiated a left turn when the aircraft lost height, stalled and crashed. A pilot was killed while the second pilots was injured.
Probable cause:
Engine failure caused by carburetor problem.
