Circumstances:
On final approach to Orenburg, the aircraft dropped below the glide path and the crew did not take the decision to attempt a go around. The three engine aircraft hit the ground 68 meters short of runway threshold with a positive aerodynamic force of 3 g, bounced and touched down again 635 meters further. On the second touchdown, the positive aerodynamic force was 3,9 g, causing the aircraft to brake in two. The airplane went out of control and eventually came to rest. Two crew members and one passenger were injured.
Probable cause:
Wrong approach configuration on part of the crew who failed to follow the approach checklist and published procedures. A series of errors and omissions on part of the flying crew led the aircraft descending below the glideslope and despite several alarms sounded, the captain failed to attempt a go-around manoeuvre. An inappropriate usage of approach charts, stabilizers at -5,5°, flaps at 45° and procedures violation caused the airplane to follow a wrong approach path. Also, the controller in charge of the approaches cleared the crew to land in lieu of giving the instruction to initiate a go-around.

Circumstances:
The pilot, sole on board, departed Moscow on a postal flight to Tashkent with intermediate stops in Kuybyshev and Orenburg. On the leg to Orenburg, the pilot encountered poor weather conditions with thunderstorm activity and heavy rain falls. He decided to continue, descended to an altitude of about 400-500 metres when control was lost. The aircraft crashed in a wheat field located 4 km south of the village of Syrt, some 40 km northwest of Orenburg Airport. The aircraft was destroyed and the pilot was killed.
Probable cause:
Loss of control while flying in stormy weather and turbulences.
The following contributing factors were identified:
- Poor flight preparation as the pilot departed Kuybyshev Airport without any information about weather conditions at destination,
- Decision of the pilot to continue in unfavorable weather conditions rather than to return to Kuybyshev or to land asap.

Circumstances:
The aircraft departed Moscow on February 19 to be delivered to its new base in Tashkent. Intermediate stops were scheduled in Penza, Samara and Orenburg. On the leg from Samara to Orenburg, after a trip of about 205 km, the crew encountered engine problems and made an emergency landing. Due to technical problems, poor weather conditions and awaiting fuel from Orenburg, the crew was stuck for 18 days. Following several engine tests and after all defaults were eliminated, the crew took off on March 11 back to Samara. While flying at a height of 250-300 metres, the engine failed again. The pilot attempted an emergency landing and initiated a turn when the aircraft stalled and crashed in a wooded area located 3 km from the Zalivnaya Station. The aircraft was totally destroyed and both crew members were killed.
Probable cause:
Loss of control consecutive to a stall during a turn completed at low altitude following an engine failure in a difficult environment.
