Chartered by the DRC Government, the airplane was carrying a delegation of the Ministry of Mines who was flying from Kinshasa to Kolwezi to meet local government following a recent incident in the mine of Kalando. After departure from Kinshasa-N'Djili Airport, a stop was made in Lubumbashi. Upon landing on runway 29 at Kolwezi Airport, both main landing gear were sheared off. The airplane slid for few hundred metres then veered off runway to the left and came to a halt perpendicularly to the runway, bursting into flames. All 29 occupants, 26 passengers and three crew members, escaped without injuries before the airplane would be destroyed by fire. At the time of the accident, the first portion of 1,000 metres of the runway 29 was in a resurfacing process. A Notam was published, informing crew that the runway 29 threshold was displaced of 1,000 metres, offering a landing distance available of 1,500 metres. The airplane overflew the portion under construction but landed before the displaced threshold, causing both main landing gear to be sheared off.

Circumstances:
Approaching Kisangani, the single engine airplane descended into trees and crashed in a dense wooded area located 34 km of Kisangani, bursting into flames. A passenger was seriously injured and six other occupants were killed. The airplane was ferried to Kisangani to be subject to a technical inspection by the Authority to officially authorize the operation of the aircraft in accordance with the regulations in force. According to Authorities, the control tower at Kisangani Airport was unaware of this flight.

The airplane departed Goma on a cargo service to Kongolo, carrying a crew of five and a load of cigarettes. About 30 km from the destination, the crew reported technical problems and continued the approach. After landing at Kongolo Airport, the airplane was unable to stop within the remaining distance. It overran in a grassy area, the undercarriage collapsed and the airplane came to rest with the right engine on fire. All five crew members escaped uninjured.

On approach to Kinshasa-N'Djili Airport, at an altitude of 11,000 feet, the crew encountered technical problems with the left engine that was shut down. The crew continued the approach and was cleared to land on runway 24. After touchdown, the crew started the braking procedure and activated the right reverser when the airplane veered off runway to the right and came into soft ground. The nose gear collapsed and the airplane came to rest. There were no injuries among the occupants.

After landing on the airfield of Tchambogo located east of the village of Katchungu, the single engine airplane went out of control, hit a small house, lost its right wing and crashed, bursting into flames. The pilot was rescued and the captain was killed. The airplane was destroyed by a post crash fire.

Circumstances:
The airplane departed Kinshasa on cargo flight to Lisala, carrying one passenger, four crew members and a load consisting of medical supplies against Malaria. On approach to Lisala Airfield, weather conditions were poor with strong winds and heavy rain falls. The airplane landed at a speed of 118 knots some 433 metres past the runway 05 threshold (runway 05 is 2,200 metres long). After touchdown, the crew initiated the braking procedure but the airplane was unable to stop within the remaining distance. It overran, lost its nose gear and came to stop in a field 172 metres past the runway end. All five occupants evacuated safely and the airplane was damaged beyond repair.
Probable cause:
It was determined that there was a sudden change of weather characteristics with nimbostratus clouds while the airplane was about 34,5 NM from the destination airport. This sudden change in weather components caused an excessive long rollout distance oof the airplane on the runway.
The following findings were identified:
- The operator (Air Kasai) did not provide the necessary documentation because their air operator's certificate and their insurance policy had been suspended.
- The aircraft's airworthiness certificate ending 23DEC2023 was forged, thus the aircraft was not airworthy.
- The 300 hour maintenance had been performed on the aircraft.
- The flight crew certificates were valid and their type ratings were up to date.
- The aircraft had not been maintained according to the manufacturer's maintenance program.
- The aerodrome's meteorologic station was inoperative due to lack of qualified personnel.
- The runway was not equipped with navigation aids.
- ATC services were not provided.
- The runway was wet and had numerous potholes.

The airplane departed Kasese Airport at 1500LT on a cargo flight to Goma, carrying two pilots, one passenger and a load of various goods for a total weight of one ton. En route, weather conditions deteriorated with low clouds and heavy rain falls. As the airplane failed to arrive at destination, SAR operations were initiated and the wreckage was found two days later in a dense wooded area located in South Kivu, about 139 km southwest of Goma. The airplane was destroyed by a post impact fire and all three occupants were killed.

Investigations were unable to determine the exact cause of the accident. Nevertheless, it may be the consequence of an insufficient consideration of weather conditions during flight preparation or during the flight itself, to a system or component failure, or to human error (error of judgment and decision-making).

As the airplane failed to arrive at destination, SAR operations were initiated and the wreckage was found two days later in a wooded and hilly terrain near Kasese. All three occupants were killed.
