Shortly after takeoff from Tsymlyansk Airfield, the single engine airplane was shot down by enemy fire and crashed, bursting into flames. The pilot, sole on board, was killed.

Circumstances:
Following an uneventful flight from Nizhneudinsk to Alygdzher, the pilot spent some time on ground with friends and has consumed alcohol. As he was preparing the return flight, people asked him to postpone the flight to the following day but the pilot said he has to return today. After takeoff, he performed several manoeuvre then climbed to the altitude of 1,600 metres. In good weather conditions, the airplane impacted trees and crashed in a wooded and hilly terrain, bursting into flames. A passenger was injured while both other occupants were killed.
Probable cause:
The decision of the pilot to initiate the flight despite he was intoxicated.

Circumstances:
The crew was performing a local training flight at Bogotol. While circling in the vicinity of the airfield, the crew modified the fuel selector from the upper fuel tank to the lower fuel tank when the engine failed. The pilot-in-command initiated a left turn to return to the airfield when the airplane lost speed. From a height of 50 metres, it stalled and crashed into the ground some 100 metres southwest from the airfield. The instructor was killed.
Probable cause:
The following findings were identified:
- Negligence on part of the chief of instruction in the surveillance of the actions of the flying instructor,
- Violation of flight manual #357, 358 & 369,
- Incorrect actions on part of the flight instructor during a turn at low height after the engine failed,
- Incorrect action on the fuel valve by the instructor, causing its closing and the engine to stop.

The airplane departed its base on a special flight to Izvaryne, carrying one pilot, one doctor and one battalion commissioner. Arriving at destination, the pilot tried to find a suitable terrain to land when, completing a last turn at low height, he lost control of the airplane that dove into the ground and crashed. All three occupants were killed. There was no hostile activities in the area at the time of the accident.
