Circumstances:
The twin engine airplane departed Belo Horizonte-Pampulha-Carlos Drummond de Andrade Airport on a flight to Setubinha-Fazenda Sequóia Airfield, carrying one employee of the company and two pilots. Prior to takeoff, the captain informed the copilot he wanted to perform an 'American' takeoff with full engine power followed by a steep climb. After liftoff, the crew raised the landing gear then continued over the runway at low height until the end of the terrain to reach a maximum speed, then initiated a steep climb at 90°. The aircraft reached the altitude of 1,700 feet in 15 seconds then stalled and entered an uncontrolled descent. It dove into the ground and crashed in a vertical attitude into a houses located in a residential area some 800 metres from the airport. The aircraft was destroyed by impact forces and a post crash fire and all three occupants were killed. One people on the ground was slightly injured.
Probable cause:
Loss of control after the crew initiated aerobatic maneuvers at low altitude.

Circumstances:
The twin engine aircraft departed Belo Horizonte-Pampulha Airport at 0700LT on a flight to Juiz de Fora, carrying six passengers and two pilots. In contact with Juiz de Fora Radio, the crew learned that the weather conditions at the aerodrome were below the IFR minima due to mist, and decided to maintain the route towards the destination and perform a non-precision RNAV (GNSS) IFR approach for landing on runway 03. During the final approach, the aircraft collided first with obstacles and then with the ground, at a distance of 245 meters from the runway 03 threshold, and exploded on impact. The aircraft was totally destroyed and all 8 occupants were killed, among them both President and Vice-President of the Vilmas Alimentos Group.
Probable cause:
The following factors were identified:
- The pilot may have displayed a complacent attitude, both in relation to the operation of the aircraft in general and to the need to accommodate his employers’ demands for arriving in SBJF. It is also possible to infer a posture of excessive self-confidence and confidence in the aircraft, in spite of the elements which signaled the risks inherent to the situation.
- It is possible that the different levels of experience of the two pilots, as well as the copilot’s personal features (besides being timid, he showed an excessive respect for the captain), may have resulted in a failure of communication between the crewmembers.
- It is possible that the captain’s leadership style and the copilot’s personal features resulted in lack of assertive attitudes on the part of the crew, hindering the exchange of adequate information, generating a faulty perception in relation to all the important elements of the environment, even with the aircraft alerts functioning in a perfect manner.
- The meteorological conditions in SBJF were below the minima for IFR operations on account of mist, with a ceiling at 100ft.
- The crew did not inform Juiz de Fora Radio about their passage of the MDA and, even without visual contact with the runway, deliberately continued in their descent, not complying with the prescriptions of the items 10.4 and 15.4 of the ICA 100-12 (Rules of the Air and Air Traffic Services).
- The crew judged that it would be possible to continue descending after the MDA, even without having the runway in sight.

Circumstances:
The twin aircraft departed Guanambi Airport to perform a low level survey flight in the region of Espinosa, carrying two observers and one pilot. About two hours into the flight, while cruising at an altitude of 330 feet, the right engine lost power then failed. While executing the emergency checklist, the left engine failed as well. The pilot attempted an emergency landing when the aircraft crashed in a wooded area, bursting into flames. Both passengers evacuated with minor injuries and the pilot was killed. The aircraft was totally destroyed by a post crash fire.
Probable cause:
There was sufficient fuel in the tanks at the time of the accident as the aircraft was refueled prior to departure for a 5-hour flight. The exact cause of the double engine failure remains unknown. When the right engine failed, the pilot was flying at an altitude of 330 feet which was below the minimum safe altitude fixed at 500 feet. Also, he was apparently using his cell phone.

Circumstances:
The aircraft departed Belo Horizonte-Pampulha on an executive flight to Manhuaçu, carrying two pilots and three passengers, among them the Brazilian singer Eduardo Costa. Following an uneventful flight, the crew started the descent to Manhuaçu-Elias Breder Airport. After touchdown on runway 02, the crew activated the reverse thrust systems but the aircraft did not decelerate as expected. So the crew started to brake when the tires burst. Unable to stop within the remaining distance, the aircraft overran, lost its undercarriage, collided with a fence and came to rest. There was no fire. All five occupants were rescued. Nevertheless, Eduardo Costa broke his nose and right hand during the accident.
Probable cause:
Late use of the normal brake systems on part of the crew after landing, causing the aircraft to overran. The captain had the habit of braking the aircraft while using the reverse thrust systems only in order to save the braking systems. Doing so, the use of the normal brakes was delayed.

Circumstances:
When the crew departed São Paulo-Guarulhos Airport, weather conditions at destination were considered as good. These conditions deteriorated en route and when the crew started the approach to Uberaba Airport by night, the visibility was below IFR minimums. Nevertheless, the crew attempted to land, continued the approach, descended below the MDA by 240 feet when the aircraft struck two houses and crashed in the district of Conjunto Pontal, bursting into flames. The wreckage was found about 800 metres short of runway 17 threshold. Both pilots as well as one people in a house were killed.
Probable cause:
The decision of the crew to descend below MDA in below weather minimums. The following contributing factors were identified:
- Low visibility (night),
- Poor judgment on part of the crew,
- Poor approach planning,
- Lack of supervision,
- The pilot acted on the controls of the aircraft to allow it to stall during the approach, losing its control and colliding with the obstacles,
- Poor crew coordination,
- Lack of discipline.

The twin engine aircraft departed Belo Horizonte-Pampulha Airport at 1522LT on a taxi flight to Juiz de Fora, carrying two pilots, two passengers and a load of valuables. While cruising about 35 nm south of Belo Horizonte, the crew encountered technical problems with the engines and elected to return for an emergency landing. On approach to runway 13, the aircraft was too high and the crew was cleared to make a 360 turn to reduce his altitude. Doing so and after he completed 270° of this turn, the aircraft entered an uncontrolled descent and crashed 2 km short of runway, bursting into flames. The aircraft was totally destroyed by impact forces and a post crash fire and all four occupants were killed.
Probable cause:
The right engine suffered a oil leak in flight following the failure of a hose due to poor maintenance. The following contributing factors were identified:
- Both pilots did not have sufficient experience nor training on this type of aircraft,
- The copilot was flying for this operator for the first time,
- The operator was unable to provide any maintenance documentation nor crew training documentation to investigators,
- Both engines were running at impact but with limited power,
- The right propeller was feathered at impact.

After takeoff, while in initial climb, the right engine lost power. The aircraft lost height and crash landed in an open field. It slid for about 100 metres before coming to rest, bursting into flames. Both pilots escaped with minor injuries and the aircraft was destroyed by fire.
Probable cause:
Loss of power on the right engine for undetermined reasons. The following findings were identified:
- Poor flight preparation on part of the crew,
- The crew did not know the exact weight of the mail/cargo,
- The crew did not have sufficient training/instruction regarding a possible loss of engine power at takeoff,
- Poor engine maintenance as the right engine had experienced power issues in the past that had not been resolved,
- Poor crew coordination,
- When the loss of power occurred on the right engine, the captain asked the copilot to raise the landing gear and preferred to resolve the emergency situation alone,
- Poor evaluation of the aircraft performances on part of the crew in regard of the weight and balance values and CofG.

Probable cause:
It is believed that the pilot lost control of the aircraft in poor weather conditions after he suffered a spatial disorientation. The following contributing factors were identified:
- Poor flight preparation,
- Poor weather conditions with atmospheric turbulences,
- The pilot showed anxiety and complacency,
- The pilot was not properly licensed to operate in IFR conditions,
- The pilot was inexperienced,
- The pilot's actions were likely inappropriate while facing an unexpected situation.

Shortly after takeoff from Belo Horizonte-Pampulha Airport, while in initial climb, the left engine failed. The pilot lost control of the aircraft that rolled to the left, lost height and crashed onto four houses located in the district of São Bernardo near the airport. The aircraft and four houses were destroyed. All three occupants were killed and two other people on the ground were injured.
Probable cause:
Failure of the left engine that was not lubricated according to published procedures. Inappropriate engine maintenance on part of the operator. Also, it was reported that the pilot's experience on this type of aircraft was insufficient.
