Circumstances:
Owned by Fenatracoop (Federação Nacional dos Trabalhadores Celestitas nas Cooperativas no Brasil), the twin engine aircraft departed Cuiabá-Marechal Rondon Airport on a flight to Londrina, carrying two pilots and six passengers, three adults and three children. On final approach to Londrina-Governador José Richa Runway 13, the pilot informed ATC about a loss of power on the left engine. Shortly later, control was lost and the aircraft crashed on a hangar housing six tanker trucks and located 9,2 km short of runway. Several explosions occurred and the aircraft and the hangar were totally destroyed. All eight occupants were killed but there were no injuries on the ground.
Probable cause:
Contributing factors.
- Communication – undetermined
It is possible that difficulties for the dialogue between pilots on matters related to the operation of the aircraft have favored a prejudicial scenario to the expression of assertiveness in the communication in the cabin, interfering in the effective management of the presented abnormal condition.
- Team dynamics – undetermined
It is possible that a more passive posture of the copilot combined with the commander's decisions and actions from the presentation of the abnormal condition in flight interfered with the quality of the team's integration and in the efficiency of the cabin dynamics during the occurrence, bringing losses to the emergency management presented.
- Emotional state – undetermined
It is not possible to discard the hypothesis that a more anxious emotional state of the pilots contributed to an inaccurate evaluation of the operational context experienced, favoring ineffective judgments, decisions and actions to manage the abnormal condition presented.
- Aircraft maintenance – a contributor
On the right engine, it was found that the fuel tube fixing nut that left the distributor for No. 3 cylinder was loose, favoring the fuel leakage, as well as the bypass valve clamp of the turbocharger that was bad adjusted, providing leakage of gases from the exhaust that would be directed to the compressor and, later to the engine, to equalize its power. On the left engine, impurity composed of an agglomerate of soil and fuel were found on the side of the nozzles n° 2, 4 and 6, which migrated to the inside of these nozzles, causing them to become clogged. It was not possible to determine the origin of this material, but there is a possibility that it may have been deposited during the long period the aircraft spent in the maintenance shop, undergoing general overhaul and the revitalization of its interior (13DEC2012 until 29APR2016).
- Insufficient pilot’s experience – undetermined
The pilots had little experience with the GARMIN GTN 650 navigation system. The lack of familiarity with this equipment may have favored the misidentification of the approach fixes for Londrina. This way, it is possible that they have calculated their descent to the final approach fix (waypoint LO013), believing that it was the position relative to threshold 13 (waypoint RWY13).
- Decision-making process – undetermined
The decision to take off from Cuiabá to Londrina without the identification of the reason for the warning light to be ON in the alarm panel and the possible late declaration of the emergency condition showed little adequate decisions that may have increased the level of criticality of the occurrence.
- Support systems – undetermined
The similarity of the waypoints names in the RNAV procedure, associated with the lack of familiarity of the pilots with the new navigation system installed in the aircraft, may have confused the pilots as to their real position in relation to the runway.

The twin engine airplane departed Miranda-Estância Caimam Airfield at 0915LT on a charter flight to Campo Grande, carrying seven passengers and two pilots. About 35 minutes into the flight, while flying 79 km from the destination in good weather conditions, the left engine failed. The crew was unable to feather the propeller and to maintain a safe altitude, so he decided to attempt an emergency landing. The aircraft belly landed in an agriculture area, slid for few dozen metres and came to rest. All nine occupants suffered minor injuries and the aircraft was damaged beyond repair.
Probable cause:
Failure of the left engine in flight due to fuel exhaustion. The following findings were identified:
- The fuel tanks in the left wing were empty while a quantity of 320 litres of fuel was still present in the fuel tanks of the right wing,
- The crew was unable to maintain altitude because he could not feather the left propeller,
- The pilots were misled by a false indication of the fuel gauge coupled to the left wing tank which displayed a certain value while the tank was actually empty. This error was caused by the fuel sensors for the left wing tanks being installed inverted,
- The aircraft was not airworthy at the time of the accident due to several defects,
- The Minimum Equipment List (MEL) was not up to date,
- The Cockpit Voice Recorder (CVR) was unserviceable,
- The automatic propeller feathering system was out of service,
- The fuel sensors for the left wing tanks had been installed inverted,
- Bad contact with the right wing fuel sensor connector plug,
- The pilots failed to follow the published procedures related to an engine failure,
- Poor flight preparation,
- Crew complacency,
- The crew training program by the operator was inadequate,
- Lack of supervision on part of the operator.

Circumstances:
The crew took off from a small airfield located in the suburb of Manaus at the end of the day with a load of 616 small packages containing cocaine. After entering the Venezuelan Airspace without clearance, the crew was contacted twice by ATC but failed to answer. Authorities decided to send two fighters to establish a visual and radio contact with the crew but without success as, due to the low visibility caused by night and poor weather conditions, the crew of the Navajo was able to continue the flight without further problem. Nevertheless, three hours later, the twin engine aircraft was again localized passing over the State of Cojedes, and the decision was taken to shoot it down. Out of control, the Navajo crashed in a pasture located in the region of Ricaurte, and was totally destroyed by impact forces. Both pilots were killed and a load of narcotics was found. At the time of the accident, the aircraft was illegally registered YV1246 as a sticker has been affixed above the official Brazilian registration PT-RCN.

Shot down by the pilot of a Venezuelan Air Force fighter.

The twin engine aircraft departed Belém-Val de Cans-Júlio Cezar Ribeiro Airport at 1907LT on a flight to Almeirim, carrying one pilot and nine employees of a company taking part to the construction of a hydro-electric station in the area. Following an eventful flight at FL85, the pilot started the descent to Almeirim and contacted ground at 2023LT. On final approach by night, the aircraft descended too low, impacted ground and crashed 5 km short of runway, bursting into flames. The aircraft was destroyed and all 10 occupants were killed.
Probable cause:
The following findings were identified:
- The pilot took the risks inherent to that flight when he accepted to be the only crewmember on a night-time flight with an aircraft in which he lacked enough experience. It is therefore considered that the pilot was complacent when he accepted to fly the aircraft under those circumstances, taking the risks associated with the operation.
- The fact that the pilot made a phone call to his father, to tell that he was feeling insecure for conducting the flight, may be considered an indication that he was not confident, and this condition may have influenced negatively his operational performance during the descent procedure.
- It is possible that the motivation of the pilot in making a fast progress in his career contributed to his acceptance of the challenge to operate the flight, even if he was not feeling fully confident.
- It is possible that the characteristics related to the type of flight, regions, time of the day, in addition to the fact that the pilot was flying the aircraft alone for the first time, contributed to an unclear perception of the relevant elements around him, leading him to a mistaken comprehension, which resulted in the deterioration of his ability to foresee the events.
- The operational progress of the pilot in the company was expedited and, therefore, it is possible that for this reason he did not gather the necessary experience for conducting that type of flight.
- It is possible that the way the work was organized within the company, with designation of pilots not readapted in the aircraft for night-time flights without artificial horizon, and for takeoffs with an aircraft weight above the one prescribed in the manual contributed to the event that resulted in the accident.
- It is possible that the prioritization of the financial sector, in detriment of operational safety, contributed to the designation of a single pilot with short experience for transporting nine passengers.
- It is probable that the pilot, during the preparation of the aircraft for landing, allowed the its speed and power to drop to a value below the minima required for maintenance of level flight on the downwind leg.
- It is possible that the location of the runway in an isolated area of the Amazonian jungle region, without visual references in a night-time flight, contributed to the pilot’s difficulty maintaining a sustained flight.
- It is possible that the training done by the pilot in a shortened manner deprived him from the knowledge and other technical abilities necessary for flying the aircraft.
- The decisions of the company operation sector to designate a short-experienced pilot without a copilot for a night flight destined for an aerodrome located in a jungle region without visual reference with the terrain increased the risk of the operation. Therefore, the risk management process was probably inappropriate.
- It was the first time the pilot was flying the aircraft on a night-time flight without a copilot. Since he had only little experience in the aircraft, it is possible that his operational performance was hindered in the management of tasks, weakening his situational awareness.
- It was not possible to determine whether the company chose to dispense with the copilot on account of the need to transport a ninth passenger and, thus, did not consider in a conservative manner the prescription contained in the aircraft airworthiness certificate by designating just one pilot for the flight.

The twin aircraft departed Guanambi Airport to perform a low level survey flight in the region of Espinosa, carrying two observers and one pilot. About two hours into the flight, while cruising at an altitude of 330 feet, the right engine lost power then failed. While executing the emergency checklist, the left engine failed as well. The pilot attempted an emergency landing when the aircraft crashed in a wooded area, bursting into flames. Both passengers evacuated with minor injuries and the pilot was killed. The aircraft was totally destroyed by a post crash fire.
Probable cause:
There was sufficient fuel in the tanks at the time of the accident as the aircraft was refueled prior to departure for a 5-hour flight. The exact cause of the double engine failure remains unknown. When the right engine failed, the pilot was flying at an altitude of 330 feet which was below the minimum safe altitude fixed at 500 feet. Also, he was apparently using his cell phone.

Probable cause:
The decision of the captain to continue the approach under VFR mode in IMC conditions to an airport that was not suitable for IFR operations. The following contributing factors were identified:
- The lack of ground references may have contributed to the commander's spatial disorientation,
- Although the weather conditions made it impossible to land under VFR conditions, the captain insisted on landing, neglecting IFR procedures,
- The captain ignored the copilot's advice and continued with the approach procedure,
- The captain put the aircraft in an attitude that caused it to stall,
- Poor judgment on part of the captain,
- Despite the implementation of a CRM program, the operator was unable to identify that the captain was violating the published procedures.

Probable cause:
The right engine suffered a oil leak in flight following the failure of a hose due to poor maintenance. The following contributing factors were identified:
- Both pilots did not have sufficient experience nor training on this type of aircraft,
- The copilot was flying for this operator for the first time,
- The operator was unable to provide any maintenance documentation nor crew training documentation to investigators,
- Both engines were running at impact but with limited power,
- The right propeller was feathered at impact.
