Circumstances:
The single engine aircraft departed Kyzylorda on a charter flight to the island of Vozrozhdeniya with an intermediate stop in Bozoy, carrying 10 doctors and two pilots. They were taking part to a plague control program. After takeoff from Bozoy, while climbing to a height of about 50 metres, the aircraft stalled and crashed in an open field. All 12 occupants were rescued, among them three passengers were seriously injured. Few hours later, one of them died from his injuries.
Probable cause:
Stall and loss of control after the cargo shifted during initial climb. It was determined that the cargo was not properly secured in the cabin and moved to the rear during initial climb. The distance between the aircraft and the ground was insufficient to expect recovery. It was also reported that the aircraft has been refueled with AI-96 motor gasoline instead of aviation fuel.

The crew continued the approach with an excessive rate of descent and at too high speed. Upon touchdown, the aircraft bounced three times then landed on its nose gear that collapsed. The aircraft slid for few dozen meters then overran and came to rest 83 meters past the runway end, bursting into flames. All 33 occupants were evacuated safely while the aircraft was destroyed.
Probable cause:
Wrong approach configuration on part of the crew who completed an approach and landing in a nose-down attitude with excessive speed and rate of descent. It was also determined that the braking systems were partially inoperative due to hydraulic failure, which was considered as a contributing factor.

In the afternoon, the crew landed on a highway instead of the designated landing area, and without assessing specific conditions, took off from it. The plane rolled for about 250 meters then took off. After a distance of 37 meters from the liftoff point, it collided with an earth mound and a gas pipe then crashed. Occupant fate unknown.

The crew was engaged in the control of gas compression stations n° 10, 11 and 12 located in the region of Shalkar. In the afternoon, the crew consumed alcohol and took off at 1745LT with two unauthorized people on board (a young couple who just got married). The crew made a short stop few minutes later to pick up three additional people who were invited to the wedding party. The crew continued the flight at an altitude of 100-150 meters. The visibility was poor because of night, mist and light rain. Several cars were parked at destination with light ON to allow the airplane to land. Unfortunately, the crew was unable to located precisely the landing area and decided to go-around. At a relative low height, the aircraft entered a nose-down attitude then banked right to an angle of 60° and crashed at a speed of 200 km/h in a field. At impact, all seven occupants were ejected and some of them were found 136 meters from the point of impact. The aircraft was destroyed and all seven occupants were killed.
Probable cause:
It was determined that the crew was intoxicated at the time of the accident. He was not authorized to perform such flight and did not have any qualification to fly by night. Also, there were five unauthorized passengers on board.

Circumstances:
The airplane was engaged in a flight from Aktyubinsk to Kenkiyak, carrying 12 passengers and a crew of two on behalf of an oil exploration company. Rotation was completed at a speed of 80 km/h after a course of 180 meters. During initial climb, at a height of 5-7 meters, at a speed of 120 km/h, the airplane banked left then lost height and crashed 210 meters from the runway end, rolled for about 26 meters and came to rest. Two passengers were killed while 12 other occupants were seriously injured.
Probable cause:
It was determined that the loss of control during initial climb was the consequence of a weight & balance problem. Investigations revealed that the aircraft was generally used for support and other missions in the past and was flying with passengers for the first time. By a local manufacturing plant, several equipment such as radios and batteries were moved to the rear of the cabin, modifying the center of gravity that was out of the envelope, too far aft (4%). For unknown reasons, the aircraft technical documents were not modified accordingly and the crew was not aware of these modifications.

The crew was engaged in several flights between Shalkar and Yuzhnyy, carrying oil field workers. After takeoff from Yuzhnyy, the crew decided to make a stop in Bozoy but there was no passengers to embark so it was decided to continue quickly. Shortly after takeoff, while flying at a relative low height in limited visibility, the single engine airplane struck a radio mast (160 meters high) located near Bozoy. The airplane embedded in the mast at a height of 85 meters. Few debris fell down and all three occupants were ejected and killed.
Probable cause:
It was determined that the crew precipitated the departure from Bozoy without checking weather conditions and flight conditions. The following contributing factors were reported:
- Poor flight planning,
- Marginal weather conditions with low clouds, patches of fog and limited visibility,
- The passenger was illegally on board,
- The crew failed to communicate with ATC,
- Insufficient altitude,
- Wrong decisions.

Circumstances:
On the leg from Baiganin to Kenkiyak of a flight from Baiganin to Aktyubinsk, the crew encountered below-minima weather conditions (low clouds and fog), but failed to return to Baiganin and descended instead so that the aircraft crashed at a height of 273 metres into a hill (288 metres) located 18 km southwest of Kenkiyak. The copilot and two passengers were killed. The captain was seriously injured and all other passengers were slightly injured. The aircraft was destroyed.
Probable cause:
It was determined that the crew decided to continue the descent to Kenkiyak in below-minima weather conditions instead of returning to Baiganin. Informations related to weather conditions and transmitted to the crew did not reflect the truth and were considered as too optimistic, which was considered as a contributing factor.

The crew departed Aralsk Airport at 0952LT on an ice reconnaissance mission over the Aral Sea. While cruising at an altitude of 800 meters, the aircraft went into clouds and the pilot decided to reduce his altitude to maintain a visual contact with the sea surface. The airplane descended to an altitude of 400 meters when it flew again in fog. The crew reduced again his altitude, causing the aircraft to struck the ground. The aircraft bounced and crashed 226 meters farther. A crew member and two passengers were killed while seven other occupants were injured. The aircraft was destroyed.
Probable cause:
It was determined that the crew continued under VFR mode in IMC conditions, causing the aircraft to struck the ground. The absence of such reconnaissance mission organization was considered as a contributing factor.

Enroute from Aktyubinsk to Uralsk, while cruising at an altitude of 1,500 meters in marginal weather conditions, the aircraft was hit by lightning. The crew lost control of the aircraft which dove into the ground and crashed in a field located 4 km southeast of Konstantinovski, in the region of Aktyubinsk. The wreckage was found few hours later and all 27 occupants have been killed.
Probable cause:
The aircraft was struck by a lightning in-flight. Discharge went through the port control cables of the left aileron located on the left wing, behind the rear spar of the center wing between the 4th and 5th section. As a result, the crew was paralyzed or in a state of shock and the control of the airplane was lost instantly. The following factors were reported:
- Presence of thunderstorm activity,
- Overheating and rupture of the control cables on the left aileron due to the exposure to high temperatures,
- The crew was unable to send any distress call,
- The dive was quick, uncontrollable and at an excessive speed,
- The crew was unable to attempt an emergency landing in night conditions,
- Employees at the meteorological station in Aktyubinsk did not indicate any thunderstorm in the weather forecast,
- The observer at the weather station of Novoalekseevka noted the presence of a thunderstorm but failed to report it to the Aktyubinsk weather station, violating the published procedures.

Built last December 2, the aircraft was on its way from the Tashkent construction plant to Aktyubinsk via Shalkar to be delivered to the Soviet Air Force. While cruising vertical to the Mugodzhar mountain range, the crew encountered poor weather conditions with low clouds and icing. The aircraft lost height and crashed on the south slope of the Mt Menshy Brat (590 meters high). The wreckage was found two days later at an altitude of 520 meters and all eight occupants have been killed.
Probable cause:
The pilot was blamed to continue the flight in such poor weather conditions, and it would be better to return to his last departure point.
