After takeoff from Moscow-Myashkovo Airport runway 06, while climbing, the left engine failed. The aircraft lost height then struck the roof of a house and crashed in the residential area of Zaozerye located 2,5 km from the airport, three minutes after takeoff. Two occupants were seriously injured while three others were killed. Few hours later, one of the survivor died from his injuries, thus a passenger was the only survivor. This model was the last LI-2 airworthy in the world.
Probable cause:
Failure of the left engine during initial climb due to fuel exhaustion. Investigations determined that the fuel selector for the left engine was positioned on an empty tank. Poor flight preparation on part of the crew. The flight engineer was intoxicated at the time of the accident.

While all occupants were uninjured, the aircraft was damaged beyond repair. The exact date of the mishap remains unknown, somewhere in June 2003.

On final approach to Moscow-Myatchkovo Airport, the crew failed to realize his altitude was too low. On short final, the aircraft struck a sand dune located short of runway threshold. Upon impact, the right main gear was torn off and the aircraft landed on its belly, slid for few dozen meters and came to rest. All four crew members escaped uninjured while the aircraft was damaged beyond repair.

While cruising in good weather conditions at an altitude of 2,700 meters and at a speed of 300 km/h, control was lost. The airplane nosed down and entered a dive. Elements from the stabilisers and from the wing tips detached during the descent and the aircraft crashed in a nose-down attitude of 15-20° and at a speed of 330 km/h in a field located 3 km southwest of Nashchekino, some 9 km off course. The airplane was totally destroyed upon impact and all six occupants have been killed. The aircraft was on its way from Tbilisi to Myachkovo with an intermediate stop in Krasnodar, carrying 20 empty gas bottle and few boxes containing metal parts for a total weight of 1,792 kg.
Probable cause:
The exact cause of the accident could not be determined with certainty. However, it was reported that the aircraft CofG was too far front and that the cargo was not properly secured. Both engines were running at impact and there was no fire prior to final impact. It is possible that the loss of control was also the result of atmospheric turbulences or an unexpected disconnection of the automatic pilot system.

About 25 minutes into the flight, weather conditions deteriorated with sudden heavy snow falls. The pilot elected to return but from a height of 150 metres, the airplane entered a dive and crashed one km north of the village of Tatiny. All five occupants were killed.

Sudden deterioration of the weather conditions with heavy snow falls and the too late decision of the pilot to return.

The airplane departed Moscow-Myachkovo Airport at 0855LT on a mail flight to Leningrad, carrying three crew members and one passenger. After takeoff, the airplane climbed to a height of 100 metres over a forest then the pilot initiated a turn to the left, apparently to return to the airport. While approaching the airfield at an altitude of 200 metres, the airplane entered a right turn then nosed down and crashed 200 metres west of the airport, five minutes after takeoff. The airplane was destroyed and all four occupants were killed.
Probable cause:
The exact cause of the accident could not be determined. However, it is believed that the pilot encountered an unexpected situation after takeoff and decided to return for an emergency landing. On final, the airplane apparently stalled due to an insufficient approach speed.

The airplane was on a ferry flight from Moscow-Myachkovo Airport to Moscow-Vnukovo Airport, carrying two crew members. It took off from Myachkovo Airport around 0930LT. Some 15 minutes into the flight, weather conditions deteriorated and the pilot got confused due the limited visibility cause by rain falls. He reduced his altitude and attempted an emergency landing in a muddy field located near Sadovniki. Upon touchdown, the airplane lost its undercarriage and was damaged beyond repair. Both crew members were uninjured.
Probable cause:
The decision of the pilot to attempt an immediate landing in an unsuitable terrain rather than returning to Myachkovo Airport. The lack of experience of the pilot in poor weather conditions remains a contributory factor as this created a confusion to him.
