Circumstances:
The aircraft departed Belém-Val de Cans Airport on a flight to Brasília with intermediate stops in Marabá and Carajás, carrying 48 passengers and a crew of six. The approach to Carajás-Parauapebas Airport was completed in poor weather conditions with rain falls, clouds down to 120 metres and a limited visibility due to rain and fog. On final the aircraft was unstable and landed hard on runway 10. On touchdown, the right main gear was torn off and the aircraft went out of control. It veered off runway to the right, collided with trees and eventually came to rest in a wooded area. The copilot was killed as the right side of the cockpit was destroyed upon impact. Ten other people were injured and 43 escaped unhurt. The aircraft was destroyed.
Probable cause:
The following findings were identified:
- Lack of crew coordination,
- The captain was overconfident,
- The copilot showed a lack of self confidence,
- The environment developed in the cockpit on approach allowed the pilots to deviate from the operational tasks primarily related to mutual controls,
- Poor approach and landing planning which required a higher sink rate than normal,
- The crew failed to make an approach briefing,
- Insufficient application of controls,
- The crew completed an unstable approach,
- Weather conditions were below minimums,
- The runway was not equipped with a lighting system but only with PAPIs,
- The crew failed to initiate a go-around procedure as the landing was obviously missed.

Circumstances:
While on a night approach to Santarém Airport, the crew decided to initiate a go-around procedure. During the second attempt to land, the crew descended below the MDA when the aircraft struck the ground and crashed 1,600 metres short of runway. A passenger was seriously injured while three other occupants were killed. It was reported that a passenger was occupying the copilot seat at the time of the accident. Also, the crew descended below MDA for unknown reasons.

Circumstances:
Five minutes after takeoff from Itaituba Airport, the crew was informed about weather conditions at destination with ceiling at ground and a visibility less than 300 metres which means that the conditions were below minimums. Upon arrival, the crew was unable to establish a visual contact with the airport and decided to circle for weather improvement. Few minutes later, the single engine aircraft crashed near the airport, killing both pilots.
Probable cause:
The accident was the consequence of the following factors:
1) Adverse Weather Conditions - Contributed
Weather conditions were unfavorable at the destination location for visual flight. There was a layer of fog near the forest.
2) Poor Instruction - Contributed
The Commander was not qualified to fly that type of aircraft and was not instrument cleared.
3) Disabled Judge - Contributed
The pilots continued the flight, even knowing the adverse weather conditions at their destination.
4) Flight Indiscipline - Contributed
One of the pilots was not qualified to fly that aircraft and, in addition, there was intentional disobedience to the air traffic rules and regulations, regarding the non observance of the minimum IFR flight level in the industry.
5) Disabled Oversight - Contributed
There was poor oversight of the responsible company by letting an unqualified pilot operate the aircraft.
6) Weak Cabin Coordination - Contributed
IFR flight without complying with IMA 100-12 rules, conducted below the limits and outside the descent profile are indications that there was no planning/coordination in the operations performed in the cockpit.

On final approach to Tucumã Airport, 3 km short of runway threshold, the engine lost power and its temperature was too high. The captain decided to attempt an emergency landing when the aircraft crash landed in a prairie. All four occupants escaped uninjured while the aircraft was damaged beyond repair.
Probable cause:
The following findings were reported:
- There was a deficiency in the instruction received, since the crew did not perform emergency training on that type of aircraft, resulting in inadequate preparation for the engine failure situation, especially at low altitude,
- There are indications that the maintenance services were inadequate and contributed to the occurrence of engine failure, due to the possible occurrence of compressor stall,
- The company performed inadequate technical and operational oversight for not providing emergency situations for the crews of that type of aircraft involved in the accident.

Circumstances:
The approach to Altamira Airport was completed by night and marginal weather conditions. Despite the visibility was limited with the presence of fog bank on final, the crew decided to continue under VFR mode. On short final, the aircraft was too low, struck trees and crashed 850 meters short of runway 07. Both pilots and 20 passengers were killed while 22 other occupants were injured.
Probable cause:
The accident was the consequence of the following factors:
- Adverse weather conditions,
- Poor approach planning,
- Poor judgement,
- Poor flight discipline as the crew failed to follow ATC instructions,
- Influence of environment as the region was conducive to the formation of fog,
- The captain had sufficient rest time but his last sleep period was less than five hours,
- The captain was going through a period in which he showed boredom, aggressiveness and demotivation.

Circumstances:
Following an uneventful flight from Goiânia, the crew started the descent to Belém-Val de Cans Airport runway 06 via heading 243° then reported 6 nm from the airport at an altitude of 2,000 feet. Eleven seconds later, the aircraft went out of control and crashed in a wooded area located on the Oncas Island, few km short of runway 06 threshold. The aircraft was totally destroyed upon impact and all four occupants were killed.
Probable cause:
It is believed that the loss of control may have been the result of windshear that existed in the approach path of runway 06.
The following contributing factors were reported:
- The crew did not have sufficient training and experience to manage a windshear situation and thus, was unable to take the appropriate corrective actions,
- Poor crew coordination,
- The captain's experience on this type of aircraft was low,
- Poor supervision,
- Lack of safety culture by the operator.
