Circumstances:
The airplane departed RAF Bottesford at 1914LT on February 14 on an operation to Milan. The following night, while flying over France on a return flight to RAF Langar, the crew noticed that all four engines' temperature was too high. Shortly later, the engine n°1 caught fire and exploded. The crew decided to abandon the aircraft but only two crew bailed out and survived while five other occupants were killed when the aircraft went out of control and crashed in a field located in Saint Brisson, west of Saulieu.

The airplane departed RAF Leconfield at 1825LT on February 14 on an operation to Cologne. Shortly after midnight, while returning to base, the crew encountered technical problems with the engines. While cruising in the vicinity of RAF Pocklington, the engines stopped, the airplane lots height then collided with trees and crashed. The air gunner was killed and four other occupants were injured.

It failed to arrive at destination and is believed to have crashed somewhere into the North Sea. Despite significant SAR operations, no trace of the airplane nor the 18 occupants was found. The airplane departed RAF Leeming at 1821LT on an operation to Cologne. On return to base, and while preparing to land, the port outer engine burst into flames and the port inner lost power. The pilot managed to gain 100 feet and five of the crew bailed out before the Halifax crash landed at 2357LT in Kirby Wiske, 6 km northwest of Soweby. The air gunner, an American, was among the five who bailed out, but he struck the ground before his parachute had full deployed and was killed.
