Circumstances:
Shortly after takeoff from Paipa-Juan José Rondón Airport, while on a flight to Medellín, the twin engine airplane, a PA-31 Panther version, went out of control and crashed in a field, bursting into flames. The airplane was totally destroyed by fire and all six occupants were killed, among them the Colombian singer Yeison Jiménez aged 34. He was performing a tour with several members of his team.

Circumstances:
The aircraft, chartered by the Colombian Army, was completing a special flight from Miraflores to Villavicencio with an intermediate stop in San José del Guaviare, carrying three crew members and a certain number of soldiers (around 25 but the exact number could not be confirmed). Shortly after takeoff from runway 20, while in initial climb, the left engine caught fire. The crew elected to return for an emergency landing and initiated a right turn when smoke entered the cabin. Shortly later, the left main gear detached and as the speed dropped, the crew attempted an emergency landing when the aircraft crashed in a wooded area located 2 km from the airport. All occupants were rescued, some were injured. The aircraft was destroyed.
Probable cause:
As the accident occurred in an area with hostile rebels, investigators were not able to reach the scene and to access to the wreckage and debris. Thus, the exact cause of the left engine failure could not be determined with certainty.

The aircraft was completing a cargo flight from Villavicencio, carrying livestock, nine passengers and four crew members. The approach to Miraflores Airport was completed in poor weather conditions. On short final, the left engine failed. The aircraft lost height, collided with trees and crashed in a wooded area. Three passengers and both pilots were killed while eight other occupants were injured.
Probable cause:
The probable cause of this accident is the loss of lift, induced by explosions and fire in the left engine, problems that the pilot was unable to overcome because he was flying under VFR mode in IMC conditions.

Circumstances:
After takeoff from Paipa-Juan Jose Rondon Airport Runway 22, the twin engine airplane encountered serious difficulties to gain height. It passed closely over trees and several houses on a distance of three km then the pilot-in-command initiated a left turn when the left wingtip struck the ground, causing the airplane to crash, bursting into flames. Five occupants were rescued while 17 others were killed. Four survivors died the next day from injuries sustained.

Probable cause:
It was determined that the total weight of the airplane was 6,491 kilos upon takeoff, some 821 kilos above the MTOW. This demonstrates negligence and carelessness in operation, in violation of the airplane's Operation Manual.

Circumstances:
The airplane departed Bogotá-El Dorado Airport at 0938LT on a cargo service to Tame, carrying three crew members, 15 passengers and a load consisting of 2,300 kilos of foodstuffs. At 1007LT, the pilot informed ATC about his position over Guateque and estimated to be over Yopal at 1030LT. This was the last radio transmission. At 1030LT, while cruising at an altitude of 3,600 metres in poor weather conditions, the airplane impacted the slope of Mt Peña de Laura. The airplane disintegrated on impact and all 18 occupants were killed.

Probable cause:
The accident was the consequence of a controlled flight into terrain caused by the combination of the following factors:
- Decision of the crew to continue the flight under VFR mode in weather conditions below minimums for such visual flight.
- The crew got lost in IMC conditions since the pilot failed to navigate properly, causing the airplane to deviate from the route mentioned in the flight plan.
- Inadequate IFR operation due to the improper use of the aircraft for instrument flight, as it did not have the necessary navigation equipment.
- Inadequate preflight inspection, as the pilot did not inspect the navigation equipment or, aware that it was out of service, began the flight under such conditions.
- Unfavorable wind, cloud cover, lack of visibility and rain in the area of ​​the trajectory followed by the aircraft.
- Deficiencies in maintenance and inspection related to the failure to technically correct the suction to the normal value, which remained very high for more than two months, potentially affecting the readings of the gyroscopic instruments.
- Authorizing the operation of the aircraft with the VOR and ADF systems that were unserviceable.
- Lack of supervision on part of the Operator over the aircraft and crew in the areas of maintenance, dispatch, operation and exploitation, allowing the owner to carry out these activities according to his criteria, which resulted in the aircraft starting the flight with the navigation equipment out of service, in violation of the Aeronautical Regulations Manual.

Probable cause:
The primary cause of the accident is an inadequate correction of a previously reported mechanical fault as oil was observed leaking into the left engine turbine section. The appropriate corrective technical action was not taken. The possible contributing factor to this accident was a material failure, consisting of the front bearing of the turbine drive shaft and accessory section failing, causing the shaft to break and therefore the total failure of the left engine immediately after takeoff, causing the loss of control of the aircraft during the turn to return to the runway.
