On approach to Anoka County-Blaine Airport Runway 09, the single engine airplane entered a sharp turn to the left then descended to the ground in a near vertical attitude before crashing onto a house located in the north part of Brooklyn Park, some 9 km short of runway, bursting into flames. The airplane and the house were destroyed by a post crash fire and the pilot, sole on board was killed. There were no casualties on the ground.

The single engine airplane departed Samedan Airport Runway 03 at 1720LT on a private VFR flight to Roskilde. On board were two passengers and one pilot. Following a straight-in climb for about two minutes, while flying over La Punt-Chamues-ch, the pilot initiated a turn to the left to enter the Albula Valley when he lost control of the airplane that crashed near houses, bursting into flames. The wreckage was found some 6 km northeast of the airport. The airplane was totally destroyed by a post crash fire and all three occupants were killed.
Probable cause:
Given the aircraft's performance and considering the weather conditions at the time of the accident, the aircraft should have reached its maximum altitude of 7,700 feet after approximately two minutes of flight. It was determined that upon takeoff, the aircraft's total weight exceeded its maximum permissible weight by 5%, while its center of gravity was beyond the aft center of gravity limit specified in the pilot's operating handbook (POH). During refueling, a fuel leak occurred at the wing overflow valves. This leak persisted during the taxi to the runway, and the pilot was alerted by flight information services. The leak ceased before the aircraft reached the threshold of runway 03.

Shortly after takeoff from Wadi Seidna AFB, in the suburb of Omdurman, northwest Khartoum area, the airplane went out of control and crashed in a residential area located near the airfield, bursting into flames. All 17 occupants were killed, among them Major General Bahr Ahmed, as well as 29 people on the ground. It is believed that the crew encountered technical problems after takeoff from the airbase.

The airplane departed Northeast Philadelphia Airport at 1805LT on an ambulance flight to Springfield-Branson Airport, Missouri, under callsign MTS56 (Med Jets 56). On board were two pilots, one pediatric patient, her mother and two paramedics. After takeoff from runway 24, the airplane continued to climb to the altitude of 1,650 feet. In marginal weather conditions (light rain and mist), the airplane entered an uncontrolled descent and crashed with a high rate of descent on Cottman Avenue, near Roosevelt Mall, about 4,7 km southwest from the end of runway 24. The airplane disintegrated on impact and several buildings and vehicles were destroyed. All six occupants were killed as well as one people on the ground.

The twin engine airplane departed Canela Airport runway 24 at 0912LT on a private flight to Jundiaí, carrying nine passengers and one pilot. Weather conditions were marginal with limited visibility due to rain falls and fog. One minute later, the airplane collided with the chimney of a private house, lost height and crashed on an inn located in the center of the city of Gramado, about 3 km west of Canela Airport. The airplane was totally destroyed as well as several buildings. All 10 occupants were killed and 17 people on the ground were injured, two seriously. The pilot and owner of the airplane, Luiz Claudio Salgueiro Galeazzi, was travelling with his wife, three daughters, sister, brother in law, mother in law and two nephews.

The crew was completing a positioning flight from Punta del Este to the airport of San Fernando in Buenos Aires. After landing on runway 05, the crew initiated the braking procedure but, for unknown reasons, the airplane was unable to stop within the remaining distance (runway 05 is 1,690 metres long). After the airplane overran the runway, the crew turned to the right to avoid runway 23 landing equipment. The airplane rolled for about 200 metres in a grassy area, crossed the perimeter fence and a small road before crashing in a street, bursting into flames. The airplane was totally destroyed and both pilots were killed. The airplane was owned by the Argentinian company New Lines, property of the businessman Jorge Pablo Brito.

The pilot reported that before the flight the airplane was fueled with 142.4 gallons of 100 low-lead fuel and that the fuel tanks contained 236 total gallons of fuel before takeoff. The local aerial survey flight was flown at 16,500 ft mean sea level (msl) and lasted about 5 hours. At the end of the survey mission and about 10 miles from the destination airport, the pilot noticed low fuel indications, then both engines experienced a total loss of power. The pilot made a right 90° turn away from the airport and selected a service road for a forced landing. During the landing roll the airplane collided with three vehicles on the road. The airplane also collided with a metal traffic light pole, which resulted in a separation of the right wing and aft fuselage. After the accident the pilot stated that the loss of engine power was due to fuel exhaustion. During the airplane wreckage recovery, about 3 gallons of fuel were drained from the left wing tanks and the right wing tanks were empty. The pilot also stated that he did not complete a detailed fuel burn calculation or a weight and balance calculation for the flight. It is likely that the airplane did not contain 236 gallons of fuel before takeoff, and the pilot was unaware of the actual fuel onboard due to inadequate preflight planning and preflight inspection.
Probable cause:
The pilot’s inadequate preflight planning and preflight inspection, which resulted in a total loss of engine power due to fuel exhaustion.
