Circumstances:
En route from Smoky Hill AFB to Tinker AFB, the crew apparently encountered an unexpected situation. While circling over Talihina, the airplane entered a flat spin and crashed in a wooded area located 10 km southwest of town, bursting into flames. All 13 crew members were killed.

Circumstances:
Shortly after takeoff from Smoky Hill AFB (Schilling AFB), while on a local training flight, an engine failed. Four crew members were able to bail out before the aircraft dove into the ground and crashed in a field located two miles southeast of Smolan, about four miles south of the airbase. The aircraft was destroyed by impact forces and nine crew members were killed. Of the four crew members who were able to bail out, three were injured and one was killed.

Circumstances:
Shortly after takeoff from Tinker AFB, the bomber went out of control and crashed on an aircraft storage area. The B-29 as well as 12 parked Republic P-47 Thunderbolt were destroyed. Two crew members were killed and five others were injured.

The crew departed Smoky Hill AFB on a ferry flight to Gulfport, first intermediate stop on a flight to Europe. En route, all four engines failed, forcing the crew to abandon the airplane that crashed in Rosedale. All 10 crew members parachuted to safety.

The airplane departed Smoky Hill AFB on a ferry flight to Morrison AFB, Palm Beach. En route, the crew encountered severe vibration with the airplane that became uncontrollable and crashed 14,5 km north of Sheridan. All nine occupants were killed.

Probable cause:
The airplane apparently had serious problems from the time it rolled off the Boeing assembly line in Seattle. It had a manufacturing issue and was condemned the day prior to the accident due to severe vibration issues.

Circumstances:
The four engine airplane departed Schilling AFB (Smoky AFB), Salina, on a flight to DeRidder, Louisiana, with a crew of 10 on board. Almost immediately after liftoff, while climbing to a height of about 75 feet, the left wing dropped and hit the runway surface, causing the airplane to crash and to catch fire. Five crew members were killed and five others were seriously injured.
