On 24 April 2020 the pilot of a Pacific Aerospace Cresco 08-600 aircraft, registered ZK-LTK (the aeroplane), was conducting agricultural flight operations spreading superphosphate fertilizer on a farm in the Kourarau Hill area, near Masterton. The airstrip was a typical topdressing airstrip, with a downward slope and a left bend of about 5 degrees partway down the strip, in the direction of take-off. The ground at the end of the airstrip dropped sharply away to a valley that ran perpendicular to the direction of the airstrip. On the commencement of the third topdressing flight, witnesses reported, the aeroplane accelerated normally from the loading point. However, the aeroplane did not follow the direction of the airstrip around to the left, as it had done during the previous two flights. Instead, the aeroplane continued in a straight line from the load point and subsequently struck uneven terrain off to the right of the airstrip. The impact with the uneven terrain caused the right main undercarriage assembly to fracture off and damage the right wing and flap. The aeroplane descended into the valley, striking a tree with the right-hand wing, then continued across the valley floor and impacted the far side of the valley, coming to rest inverted. The aeroplane was destroyed by the impacts and a post-crash fire. The sole pilot occupant did not survive the accident sequence.
Probable cause:
The following findings were identified:
- The pilot did not make the necessary left turn during the take-off roll to align with the strip centreline, so the aeroplane continued the take-off roll in a straight line.
- The right-rear undercarriage struck uneven ground with sufficient force to break the undercarriage mounting brackets and dislodge the undercarriage assembly from the aeroplane.
- The right main undercarriage subsequently struck the right flap, resulting in a partial dislocation of the flap. This very likely resulted in the aeroplane becoming uncontrollable.
- The pilot was about as likely as not to have been incapacitated early in the take-off roll.

Circumstances:
The pilot and the passenger were completing a top dressing mission in the region of Otane, south of Hastings, New Zealand. On final approach, the single engine aircraft seemed to be too low and hit tree tops before crashing nose down in a prairie. Both occupants were seriously injured and the aircraft was destroyed. It was dark at the time of the accident as the sunrise was computed at 0639LT.

Circumstances:
On Sunday 14 December 2008, the aircraft departed from Stratford Aerodrome at 0630 hours for a transit flight to a farm airstrip near Tarata. Shortly after becoming airborne the pilot noticed the engine chip detector warning light on the instrument panel was illuminated. He diverted to the company maintenance base at Wanganui aerodrome for the defect to be rectified. The aircraft engineer found a light metallic fuzz on the engine magnetic (mag) plug. The fuzz was cleaned off and the mag plug refitted. The aircraft was released to service with a condition that a further inspection of the mag plug was to be performed after 10 hours flight time. The topdressing job, which involved the spreading of 450 tonnes of lime, had commenced on Thursday 11 December 2008 and continued on Friday 12 December 2008. No flying took place on Saturday 13 December 2008 due to a local horse-riding event being held on the farm property. The aircraft arrived at the farm airstrip at 0940 hours on the Sunday morning, and shortly thereafter commenced operations to complete the spreading of the lime. At the time of the accident, 423 tonnes of lime had been spread. The pilot flew a series of topdressing flights before needing to stop for the first refuel. When interviewed, the loader driver stated that the pilot informed him that he was having some difficulty with the lime product not flowing consistently from the aircraft hopper during the sowing runs. At approximately 1145 hours the pilot stopped again to refuel. On completion of the refuel, this gave the aircraft an estimated fuel load of 300 litres. The pilot completed a further two flights. On the third flight, the aircraft became airborne at the end of the airstrip and then descended 55 feet below the level of the airstrip where the aft fuselage struck a fence line. A concentration of lime along the aircraft’s take-off path indicated that the pilot had initiated an attempt to jettison his load at the end of the airstrip. Following the collision with the fence, the aircraft remained airborne for a further 450 metres before it impacted the side of a small hill in a slight nose down attitude. The aircraft then came to rest 12 metres to the left of the initial impact point. The accident occurred in daylight, at approximately 1155 hours NZDT, at Tarata, at an elevation of 410 feet amsl. Latitude: S39° 08.169', longitude: E174° 21.710'.
Probable cause:
Conclusions:
- The pilot was appropriately licensed, held the appropriate Medical Certificate, was experienced and fit to carry out aerial topdressing operations.
- The aircraft had been operating normally from the airstrip up to the time of the accident.
- The aircraft descended after take-off and struck a fence. The collision with the fence damaged the elevator control cable system which jammed the elevator control surface. This resulted in the pilot being unable to adequately control the aircraft in pitch, and the aircraft subsequently struck the ground.
- The aircraft was loaded with 1900 kg of lime product on the accident flight, this was in excess of the 1860 kg maximum structural hopper load. No variation above the maximum structural hopper load is allowed for in CAR Part 137. The aircraft’s all-up weight at the time of the accident was under the maximum allowed under the overload provisions of CAR Part 137 by 145 kg.
- The Aircraft Flight Manual does not provide take-off performance data for operation over the maximum certificated take-off weight and up to the maximum agricultural weight as allowed by CAR Part 137.
- A change in wind direction had occurred in the late morning which may have presented the pilot with a slight tail-wind or possible low level turbulence, including down draught conditions, during and after take-off.
- The windsock was not in the most suitable position to indicate the wind conditions to the pilot.
- Partial or full load jettisons had taken place on previous flights, indicating that the pilot was having difficulty achieving the required aircraft performance during or after take-off.
- On the accident flight, the aircraft was probably overloaded for the prevailing environmental conditions.
- The reported poor flowing qualities of the lime product being spread may have hampered the pilot’s efforts to jettison the load after take-off. The effectiveness of the jettison may have also been reduced by the downward flight path of the aircraft on leaving the end of the airstrip. It is unlikely that the pilot could comply with the CAR Part 137.103 requirement to jettison 80% of the load within five seconds.
- The possibility of a pre-existing airframe or engine defect that could have contributed to the accident was eliminated as far as practicable by the investigation.
- The ELT fitted to the aircraft was no longer an approved type, therefore the aircraft was not airworthy in accordance with CARs. The ELT was incapable of being detected by satellite and therefore would not automatically alert rescue services, however, this did not hamper rescue efforts in this accident.
- The accident was not survivable.

The pilot was ferrying the aircraft under the visual flight rules (VFR) from the operator’s base at Tully, Qld to Ingham and return. The flights, conducted in the private category without passengers, were to allow aircraft maintenance to be conducted at Ingham. The flight from Tully to Ingham was conducted in the morning, with no reported difficulties. At 1454 Eastern Standard Time, the pilot departed Ingham on the return flight to Tully. The aircraft did not arrive at Tully. It was not until the next day that the pilot and aircraft were reported missing. Australian Search and Rescue (AusSAR) was notified and a search, based on the last air traffic control radar observed position of an unidentified aircraft from a replay of recorded radar data together with witness reports from the area, was initiated. Searchers located the aircraft wreckage on the morning of 18 August. The aircraft had impacted mountainous terrain in a state forest 24 km south of Tully. The pilot was fatally injured and the aircraft was destroyed.

Contributing safety factors:
• The aircraft probably entered an area of weather that deteriorated below visual meteorological conditions and for which the pilot was not experienced or qualified.
• The pilot probably became unsure of his position in poor visibility, leading to controlled flight into terrain, fatally injuring the pilot and destroying the aircraft.
Other safety factors:
• The aircraft had not been configured for poor visibility operations, possibly increasing the pilot’s difficulty in navigating.
• The pilot did not submit any form of flight notification such as a SARTIME or Flight Note, as required for a flight in a designated remote area, resulting in a delay to the search and rescue response.
• The operator did not have procedures to provide assurance that a search and rescue would be initiated in a timely way if one of their aircraft did not arrive at the planned destination. [Safety issue]
• As a result of damage to the emergency locator beacon antenna, the beacon did not alert search and rescue organisations to the aircraft accident.

On 13 June 2001, Cresco ZK-TMO arrived at Te Aroha Station, 25 km west of Gisborne, to carry out topdressing of that property. Before operations started, the property owner briefed the pilot, and two observation flights over the areas to be sown were made. The owner also cautioned the pilot against turning left after take-off from the airstrip. Spreading commenced at 1600 hours, and 14 flights with 1800 kg loads were completed that day. The loader driver reported that no difficulties were experienced during those operations. Spreading recommenced at 0720 hours on 14 June 2001, again with 1800 kg loads, in an area to the left (east) of the airstrip. On the third flight after a refuel later in the morning, the loader driver noticed that the pilot made a partial load jettison after take-off; this resulted in a reduced duration of that sortie. However, on return, the pilot did not indicate to the loader driver that he required a reduced load. About 1145 hours, the aircraft was reloaded and commenced take-off. The loader driver watched about half the take-off roll, then continued preparing for the next load. He did not sight the aircraft again before a pall of black smoke attracted his attention. After making a radio call to the aircraft and hearing no response, the loader driver ran towards the smoke, which was to the left of and below the elevated strip. On reaching the site he found the aircraft inverted in a small stream and burning fiercely. The loader driver could see the pilot inside the aircraft but he and the property owner were unable to reach him because of the intense heat of the fire. The accident occurred in daylight, at approximately 1145 hours NZST, at Te Aroha Station; latitude S 38° 38.0' longitude E 177° 41.8', at an elevation of approximately 770 feet.

The following findings were identified:
- The pilot was appropriately licensed, rated and fit for the flights being undertaken.
- The aircraft had a valid airworthiness certificate and had been maintained in accordance with current requirements.
- No pre-accident defect was found with the aircraft.
- The pilot had turned left after take-off from the strip, against the advice of the property owner.
- The advice was given in light of a previous accident in virtually identical circumstances.
- There was insufficient space available after take-off for the aeroplane to accelerate to a speed at which the bank angle necessary to clear the terrain could be sustained in level flight or a climb.
- The accident was not survivable.
