Circumstances:
The single engine aircraft was engaged in a military training exercise with 2 pilots and 13 soldiers (paratroopers from the Special Forces) for a routine local paratroop flight. Few minutes after takeoff, the crew decided to return to Aranchi Airport because weather conditions deteriorated. On final approach, the crew encountered local patches of fog and in a reduced visibility, the aircraft descended too low and impacted ground few hundred metres short of runway. The aircraft was destroyed and all 15 occupants were killed.

Following an uneventful flight from Termez, the crew started the descent to Tashkent-Yuzhny Airport by night and marginal weather conditions. The visibility was limited due to foggy conditions with an RVR between 600 and 900 metres for runway 08L. The captain continued the approach with an excessive rate of descent, causing the aircraft to pass below the MDA without any visual contact with the ground. At an altitude of 165-170 metres, the captain positioned the airplane in a flat attitude then continued the descent at a distance of 2 km from the runway threshold, but this time with an insufficient rate of descent. The aircraft passed over the runway threshold at a height of about 30-40 metres and flew over the runway for a distance of 3,3 km. The captain established a visual contact with the runway lights, elected to land but failed to realize he was in fact approaching the end of the runway which is 4 km long. He reduced both engines power to idle, activated the thrust reversers when he realized his mistake and attempted a go-around. The aircraft collided with a 2 metres high concrete wall located 260 metres past the runway end, lost its right wing and crashed in a drainage ditch located along the perimeter fence, bursting into flames. The aircraft was totally destroyed and all 37 occupants were killed, among them Richard Conroy, special UNO representative in Uzbekistan.
Probable cause:
The following factors were identified:
- The crew failed to maintain a correct approach pattern maybe following a wrong setting of the approach selector in SP mode instead of ILS mode,
- The crew decided to continue the approach without establishing any visual contact with the approach light and runway light system,
- The crew failed to comply with published procedures,
- The crew failed to initiate a go-around procedure.

Following a wrong approach configuration, the crew passed over the runway threshold at a height of 30 meters and an excessive speed of 350 km/h. Rather than initiating a go-around procedure, the pilot-in-command continued and the aircraft landed 710 meters past the runway threshold. Unable to stop within the remaining distance, the aircraft overran at a speed of 225 km/h, lost its undercarriage then slid for about 317 meters and eventually collided with concrete blocks (2 meters high), bursting into flames. 29 passengers were injured while 34 other occupants were killed, among them all four crew members.
Probable cause:
Wrong approach configuration on part of the flying crew (too high on the glide and excessive speed) who failed to initiate a go-around while the landing procedure was obviously missed. The concrete blocks struck by the airplane were part of construction work for the new runway. The following contributing factors were reported:
- The crew violated/neglected various procedures,
- Lack of crew discipline,
- Poor crew coordination,
- The crew failed to use all available braking systems.

The crew was cleared for takeoff from Tashkent-Yuzhny Airport and started the procedure. After liftoff, while climbing to a height of 15-20 meters, at a speed of 230 km/h, the aircraft banked left, lost height and crashed, bursting into flames. The aircraft was destroyed and all nine occupants were killed. The aircraft was on its way to Shakhrisabz, carrying five passengers, four crew members and a load of 1,200 kilos of mail and various goods.
Probable cause:
The loss of control during initial climb was the consequence of wake turbulences created by an Ilyushin II-76 (CCCP-76482) that just took off from the same runway. It was determined that the time between both takeoffs was 81 seconds only. ATC was blamed to clear the Yak-40's crew too early and not observing the minimum separation between both takeoffs. It was also reported that loading of the cargo was longer than expected and that the Yak-40's crew hurried the departure, neglecting several procedures.

Shortly after takeoff, on a short leg from Chirchik AFB to Tashkent-Vostochny (27 km between both airfields), the aircraft was too low and crashed at an altitude of 980 meters on a mountain slope located about 15 km south of Chirchik AFB. The aircraft was destroyed and all six occupants were killed.
Probable cause:
It was reported that the crew fed the air pressure of Tashkent-Vostochny too early into the altimeters and lost his orientation after takeoff.

The approach and landing at Tashkent-Yuzhny Airport were completed in marginal weather conditions with strong crosswinds. After touchdown, the airplane went out of control, veered off runway and came to rest. All 8 crew and 89 passengers escaped unhurt while the aircraft was damaged beyond repair.

Just after liftoff, while in initial climb, the airplane stalled and crashed back onto the runway, veered off runway and came to rest in flames. All occupants escaped uninjured while the aircraft was destroyed.
Probable cause:
The total weight of the aircraft was above MTOW at the time of the accident and the airplane has been refueled with a too low octane fuel, which caused the engines to lose power during takeoff.

During the takeoff roll, at a speed of 230 km/h, the engine n°4 failed. The captain decided to abandon the takeoff procedure and started an emergency braking maneuver. Unable to stop within the remaining distance, the airplane overran at a speed of 100 km/h, went through a road and a ditch before coming to rest, broken in two. 20 passengers were injured, three of them seriously as they were not wearing their seat belt. Few hours later, one of them died.
Probable cause:
It was determined that the engine n°4 failed after colliding with birds during the takeoff roll, causing the high pressure turbine to fail. Several errors on part of the flying crew contributed to the accident, such as a wrong decision on part of the captain, his lack of knowledge about the time needed between the engine full power mode and the activation of the reverse mode. Also, the thrust reverse systems were activated on both left engines only as the flight engineer failed to activate the thrust reverse system on engine n°3.

Circumstances:
During a crop-spraying flight for the kolkhoz (collective farm) im. Kalinina in the Buka district of the Tashkent region (Uzbekistan), while flying at a height of 10 meters, the single engine airplane collided with a high-voltage power line located between the villages of Dzharkishlak and Kashka (near Almalyk). Out of control, it crashed in a cotton field 94 metres further on and nosed over. The captain was killed instantly while the copilot died while being transferred to the hospital. It was determined that the accident occured while the crew was spraying a field which has not to be sprayed.
Probable cause:
Lack of discipline on part of the crew who failed to realize he was flying at an insufficient altitude and failed to locate the high-voltage power line.
