On the leg from Karaganda to Rostov-on-Don of a flight from Almaty to Simferopol when probably the autopilot system failed while the aircraft was flying at a height of 7,800 metres by night. The aircraft banked to the left, entered a dive and was recovered by the crew at a height of some 5,000 metres. The crew managed to land safely at Rostov-on-Don, but the aircraft suffered structural damage during the dive with up to 3.5 g forces, and was damaged beyond repair. There were no casualties. SOC 29sep71 as worn out.
Probable cause:
Loss of control in flight caused by a probable failure of the autopilot system.

Probable cause:
The crew forgot to extend the flaps in an appropriate position for the takeoff procedure, causing the aircraft to stall at liftoff.

The crew was completing a crop spraying flight on behalf of the kolkhoz 'Krasny Partizan' located north of Taganrog, in the Rostov oblast, without any ATC authorization. En route, the crew encountered poor weather conditions (below minima) with very low visibility due to thick fog. He suffered a spatial disorientation and lost control of the airplane that crashed in a snow-covered field located some 5,750 metres west of Nikolayevo-Kozlovski, while completing a right turn at low height. Both pilots were killed.
Probable cause:
Loss of control after the crew suffered a spatial disorientation in flight.

Circumstances:
The aircraft was engaged in a crop-spraying flight for the sovkhoz (state farm) "Novo-Zolotovski" in the Semikarakorsk district of the Rostov region with one pilot and a local guide on board. While at a height of 20-25 metres, the pilot probably got distracted by the passenger and lost control of the airplane that descended and struck the ground, caught fire and burnt out. While the passenger was killed, the pilot was injured.

Circumstances:
While cruising at an altitude of 8,000 meters in good weather conditions, the four engine airplane went out of control, entered a dive and reached an angle of 107° and a speed of 630 km/h before crashing in a snow covered field located 10 km east of Chebotovka, in the Rostov region. The aircraft disintegrated on impact and all 59 occupants were killed.
Probable cause:
It was determined that the loss of control was the consequence of an error on part of the flight mechanic who inadvertently positioned the flaps in an angle of 40° while in cruising flight. This reduced the longitudinal stability and the control was rapidly lost. It was reported that the flap lever was not protected against accidental actuation which was considered as a contributing factor.

Circumstances:
The airplane departed Myachkovo Airport on a mail flight to Krasnodar with intermediate stops in Borisoglebsk, Morozovsk and Rostov-on-Don. On board were four crew members, one passenger and a load of 174 kilos of military mail. While cruising along the Don River and approaching a lock in Kotchetovskaya at a height of 25-30 metres, the airplane collided with a guyed mast that severed the left wing. Out of control, the airplane entered a dive and crashed 1,5400 metres from the point of impact, bursting into flames. All five occupants were killed.
Probable cause:
Failure on part of the pilot to see and avoid a guyed mast while cruising at an insufficient altitude. Cruising facing the sun was a contributing factor.
