The airplane, a Boeing 787-8 Dreamliner, flight AI171, departed Ahmedabad-Sardar Vallabhbhai Patel International Airport on a regular schedule service to London-Gatwick, carrying 230 passengers and 12 crew members. After takeoff from runway 23, the airplane climbed to an altitude of 625 feet with the undercarriage still down. Then it started to descend, first in a relative flat attitude, then nosed up (increasing the angle of attack) until it collided with several residential buildings located near Ghoda Camp Road, some 1,500 metres from the runway end, bursting into flames. The airplane was totally destroyed by impact forces and a post crash fire. Weather conditions at the time of the accident were considered as good. It appears that the crew issued a mayday call shortly after liftoff. Few hours after the crash, local authorities confirmed that one passenger survived while 241 other occupants were killed as well as 19 people on the ground. 67 other people on the ground were injured. Runway 23 is 3'600 metres long and it seems that the rotation was completed very late. An analysis of the Airport CCTV shows that the airplane ceased to climb about 12 seconds after rotation, and that the final impact occurred some 30 seconds after liftoff. It appears that the RAM Air Turbine (RAT) was deployed at the time of the accident, suggesting issues with engines.
Probable cause:
In the below preliminary report, its is reported that the aircraft achieved the maximum recorded airspeed of 180 Knots IAS at about 08:08:42 UTC and immediately thereafter, the Engine 1 and Engine 2 fuel cutoff switches transitioned from RUN to CUTOFF position one after another with a time gap of 01 sec. The Engine N1 and N2 began to decrease from their take-off values as the fuel supply to the engines was cut off. In the cockpit voice recording, one of the pilots is heard asking the other why did he cutoff. The other pilot responded that he did not do so.

Circumstances:
The aircraft departed Mehran NAS in Karachi on a survey flight over Kashmir and the Pakistan/India border. On board were 16 crew members, six officers and 10 marines attached to the Pakistan Navy Corps based at Mehran NAS, Karachi. In flight, the aircraft was shot down by an air-air missile shot by the pilot of an Indian MiG-21 fighter. The Breguet entered a dive and crashed in the Rann of Kutch, Indian territory. The aircraft disintegrated on impact and all 16 occupants were killed.
Probable cause:
The aircraft was shot down by an air-air missile from an Indian MiG-21 fighter after it entered illegally the Indian airspace.

The aircraft was engaged in a training flight at Baroda airport. It was under the command of an examiner pilot with the trainee pilot on right hand seat. The first touch and go exercise was uneventful. During the second exercise, immediately on touchdown the left main landing gear failed at its shock strut outer cylinder and the inner cylinder with the main wheel assembly attached to it got liberated. The aircraft continued moving forward with the remaining portion of left shock strut contacting the runway followed by the left engine propellers and fuselage bottom and finally came to rest on the runway left side strip. There was no fire and no injury to persons on board the aircraft.
Probable cause:
The accident occurred during aircraft touchdown due to liberation of the sliding piston along with the wheels of the left main landing gear as a result of failure of the locking arrangement on the piston top end. Non-compliance by the operator of the Service Bulletin issued by the Manufacturer and Mandatory Modifications issued by the DGCA regarding piston-adapter and dowel pin lock fitment, is the Contributory Factor in the accident.

Indian Airlines B-737 aircraft VT-EAH was operating scheduled flight IC113 (Bombay - Ahmedabad sector) on 19-10-1988. There were 135 occupants on board including 6 crew members. Aircraft contacted Ahmedabad Approach Control at 0620LT. Ahmedabad weather as per Metar of 0540LT was passed to the aircraft and again at 0625LT (speci) of 0610LT was passed to the aircraft and then visibility has been reduced from 6 km to 3 km. Clearance to descend to FL150 was given at 0632LT and the aircraft was further cleared to FL55, 25 DME at 0636LT. At 0641LT, the aircraft was advised to report over Ahmedabad VOR and 'Speci' of 0640LT was also transmitted to the aircraft as per the same the winds were calm, visibility 2 km in haze and was 1010. QNH was correctly read back by the aircraft. The pilot decided to carry localiser-DME approach for runway 23 and reported overhead Ahmedabad. The aircraft went outbound and reported turning inbound. This was the last transmission from the aircraft and thereafter contact with the ATC was lost at 0650:53. The aircraft was found crashed at a distance of 2,540 meters from the beginning of runway 23 on the extended centre line of the runway in a paddy field at about 0653LT. Out of 135 persons on board, 133 received fatal injuries. The remaining two were seriously injured. The aircraft was destroyed due to post impact fire.
Probable cause:
The cause of the accident is attributed to error on part of the pilot-in-command as well as copilot due to non adherence to laid down procedures under poor visibility conditions. Both pilot and copilot failed to follow approved procedures, directives, instructions, etc. Weather was considered as a contributing factor.

While approaching Mithapur from the north, the twin engine aircraft was shot down by the pilot of a Pakistan Air Force F-86 Sabre, dove into the ground and crashed in a huge explosion in a field located in Suthri. The aircraft was totally destroyed and all seven occupants were killed, among them Balwantrai Mehta, Chief Minister of the Gujarat State who was flying with his wife. At the time of the accident, the airplane was off course for undetermined reason but Indian Authorities reported the aircraft did not penetrate the Pakistan Airspace.

Shot down by the pilot of a Pakistan Air Force fighter.
