Circumstances:
After the airplane has been leased for the summer period for forest fire survey missions, the contract was ended early October and the airplane was ferried back from Verkhneusinskoye to Shushenskoye. The crew departed Verkhneusinskoye at 1605LT and maintained regular radio contacts with their base, confirming that weather conditions were good. Nevertheless, while at halfway-point, weather conditions deteriorated. While cruising in clouds, the airplane collided with tall pine trees and crashed in a dense wooded area located some 40 km southwest of Tanzybei, bursting into flames. The airplane was destroyed by impact forces and a post crash fire and both crew members were killed.
Probable cause:
The accident resulted from the PIC's loss of visual contact with ground references when entering non-VFR weather conditions, resulting in a collision with obstacles and the ground.
The accident was the result of a combination of the following factors:
- The crew's unfounded decision to conduct a VFR flight along a route through mountainous terrain, without analyzing all necessary meteorological information.
- Failure to abort the flight and to return to the departure landing site when weather conditions deteriorated below established limits.
- Flight below safe altitude in conditions of limited visibility.

Circumstances:
The pilot, sole on board, departed Morozov at 0708LT to perform a crop spraying flight on behalf of the Rodnik Farm. About two minutes later, while cruising at low height, the undercarriage collided with a 3-meter high embankment. The airplane flipped over and crashed inverted in a drainage ditch. The wreckage was found about 4 km south of the village. The airplane was destroyed and the pilot was killed.
Probable cause:
The accident was due to a collision of the main landing gear wheels with the embankment of an irrigation canal, which led to the destruction of the landing gear structure, the subsequent collision of the aircraft with the water surface of the canal, and a nose-over.
The accident was the result of a combination of the following factors:
- Loss of the captain’s working capability in flight due to acute cardiovascular insufficiency that developed as a result of a recurrent myocardial infarction against the background of hypertension.
- Possible influence on the captain’s health of the presence of ethyl alcohol in his blood.
- Performing the flight with an incomplete crew.
- The death of the captain in the aviation accident may have been facilitated by the absence of seat belts on the crew seats.

Circumstances:
The airplane departed Khabarovsk on a schedule service to Tynda (flight 2G/AGU2311) with an intermediate stop in Blagoveshchensk, carrying 42 passengers (including five children) and six crew members. It departed Ignatyevo Airport at 1121LT on the second leg to Tynda. After the crew started the descent to the destination airport, he was cleared for a NDB approach via LMM and a landing on runway 06. Weather at destination was marginal with the base of scattered clouds at 210 metres. After the altimeters were set, the crew continued the approach when the radio altimeter alarm sounded. Three seconds later, the airplane collided with trees and crashed in a wooded area located 14 southwest of the airport, bursting into flames. The airplane was destroyed and all 48 occupants were killed.
Probable cause:
The accident resulted from a discrepancy between the altitude reference level based on the QNH pressure actually set on the barometric altimeters and the reference level used by the crew for assigned flight altitudes (based on QFE). This led to the aircraft flying approximately 600 meters below the assigned altitude, colliding with treetops and the ground.
The accident was the result of a combination of the following factors:
- The crew’s actual lack of preparedness to operate to aerodromes that had transitioned to QNH, which led to errors in the use of aerodrome pressure values (QNH/QFE) at Tynda aerodrome.
- The controller transmitting the QNH value, expressed in millimeters of mercury, without a request from the crew.
- The crew’s failure to monitor the consistency between the barometric pressure settings used and the assigned altitude values.
- The inability of ATC to monitor the aircraft’s flight altitude instrumentally due to inoperative secondary radar equipment at Tynda aerodrome.
- The crew’s deactivation of the aural Ground Proximity Warning System (GPWS) alert.

The pilot decided to return to the airport and initiated a turn to the left. Too low, the airplane collided with a lighting mast located on the bridge of the Suvorovskoye shosse road leading over the A-157 highway. The airplane continued to fly for some 100-150 metres before coming down into trees, about one km west of the airfield. All nine occupants were quickly rescued. The airplane was damaged beyond repair.
Probable cause:
Loss of engine power caused by a magneto malfunction. The accident was the result of non-compliance with established requirements during aircraft maintenance.

Circumstances:
The single engine airplane was engaged in an on-demand charter flight from Olyokminsk to Shushenskoye with an intermediate stop in Zheleznogorsk-Ilimsky. On board were three employees of a gold mine company and two pilots. After takeoff from Olyokminsk Airport, while climbing to a height of 70-100 metres, the crew noticed that the exhaust gas temperature was abnormally high (570°C). The copilot reduced the engine power but the temperature did not decrease. Suddenly, the engine lost power and as the speed started to decrease, the airplane started to descend so the pilot attempted an emergency landing when the airplane crashed in a dense wooded area located 600 metres east from the runway 26 threshold. A passenger was killed and four other occupants were injured. The airplane was destroyed.
Probable cause:
Loss of engine power in flight caused by the failure of the exhaust gas temperature regulator (RT12-7 IIc). This led to a subsequent loss of airspeed, a descent, and the aircraft’s collision with trees and the ground.
The accident resulted from a combination of the following factors:
- Violation of the regulator repair procedures.
- Repairs of the regulator were carried out by personnel who did not have specific training.
- The captain failed to check the proper functioning of the temperature regulator after the engine was started, as required by the Aircraft Flight Manual (AFM).
