Circumstances:
The airplane departed Khabarovsk on a schedule service to Tynda (flight 2G/AGU2311) with an intermediate stop in Blagoveshchensk, carrying 42 passengers (including five children) and six crew members. It departed Ignatyevo Airport at 1121LT on the second leg to Tynda. After the crew started the descent to the destination airport, he was cleared for a NDB approach via LMM and a landing on runway 06. Weather at destination was marginal with the base of scattered clouds at 210 metres. After the altimeters were set, the crew continued the approach when the radio altimeter alarm sounded. Three seconds later, the airplane collided with trees and crashed in a wooded area located 14 southwest of the airport, bursting into flames. The airplane was destroyed and all 48 occupants were killed.
Probable cause:
The accident resulted from a discrepancy between the altitude reference level based on the QNH pressure actually set on the barometric altimeters and the reference level used by the crew for assigned flight altitudes (based on QFE). This led to the aircraft flying approximately 600 meters below the assigned altitude, colliding with treetops and the ground.
The accident was the result of a combination of the following factors:
- The crew’s actual lack of preparedness to operate to aerodromes that had transitioned to QNH, which led to errors in the use of aerodrome pressure values (QNH/QFE) at Tynda aerodrome.
- The controller transmitting the QNH value, expressed in millimeters of mercury, without a request from the crew.
- The crew’s failure to monitor the consistency between the barometric pressure settings used and the assigned altitude values.
- The inability of ATC to monitor the aircraft’s flight altitude instrumentally due to inoperative secondary radar equipment at Tynda aerodrome.
- The crew’s deactivation of the aural Ground Proximity Warning System (GPWS) alert.

Crash of a Yakovlev Yak-40 in Aldan

Circumstances:
Twenty-three minutes after takeoff from Tynda Airport, while cruising at an altitude of 5,700 metres, the crew encountered technical problems with the engine n°2 (central engine). Due to an excessive temperature of the turbine, the captain decided to shut this engine down (the engine n°2 is equipped with a thrust reverser system). While descending to Aldan Airport, the aircraft approached in a nose down attitude and struck the ground 90 metres short of runway threshold with a positive acceleration of 4 g. On impact, the nose gear collapsed, the aircraft bounced several times then slid for 800 metres before coming to rest on the runway. All 11 occupants evacuated safely while the aircraft was damaged beyond repair.
Probable cause:
The following findings were reported:
- The crew took the wrong decision to fly to Aldan Airport instead of the alternate,
- The approach was continued with excessive gap related to the approach speed and altitude,
- On final, the aircraft was in an excessive nose down attitude,
- The crew failed to identify and correct the wrong approach configuration.

Circumstances:
An Aeroflot LI-2 registered CCCP-L4279 made an emergency landing near Tynda on April 20. A day later, a Deputy from the Amur Region decided to conduct a mission to help and forced the crew to takeoff despite the fact that weather conditions were below minima. The aircraft left Takhtamygda Airport at 1550LT. About twenty minutes later, the right engine failed. The crew was forced to feather the propeller and the captain decided to return to his departure point. Shortly later, while losing altitude, the aircraft hit tree tops with its propeller while the left elevator hit a telephone pole. The aircraft stalled and crashed. Three crew members, among them the captain, were killed while three other occupants were injured.
Probable cause:
Failure of the right engine in flight. The crew was forced to conduct the mission by a local Deputy despite the fact that weather conditions were below minima at the time of the accident.
