Circumstances:
On approach to New Haven-Tweed Airport, the engine failed. The pilot lost control of the aircraft that crashed on the ground. All four occupants were killed.
Probable cause:
Loss of engine accessory drive due to excessive wear of splined gear drive. The pilot-in-command failed to maintain flying spee.

Circumstances:
The crew departed Dallas-Love Field Airport at 1910LT on a night training flight. The airplane landed first in Oklahoma City then returned to Dallas in the late evening. During the final approach from the northwest for a landing on the northwest-southeast
runway, the control tower warned the pilot that his approach was too low on account of an unlighted power line near the airport. The instructor acknowledged the warning and immediately applied power to gain altitude. Simultaneously, the airplane struck the 3 -wire power line and crashed on the embankment of a railroad beyond. All three occupants were injured and the airplane was extensively damaged.

Probable cause:
Presence of an unlighted obstruction, of which the pilot was unaware, in the approach area to the airport runway.
The following contributing factor was identified:
- Unnecessarily low approach in view of darkness and the pilot's unfamiliarity with existing obstruction.
