Circumstances:
The airplane departed Kiev-Borispol Airport at 2321LT on the second leg of a flight from Odessa to Kazan. While cruising by night at an altitude of 9,000 meters and a speed of 530 km/h, the flaps extended to an angle of 30°. The airplane entered a spin and partially disintegrated at an altitude of 4,000 meters before crashing in a field located near Kirsanov. The aircraft was totally destroyed and debris were found on a wide area. None of the 63 occupants survived the crash.
Probable cause:
It was not possible to determine the exact cause of the extension of the flaps to an angle of 30° while the aircraft was in normal cruise with the autopilot system engaged. Various analysis could not define if the extension was the result of an electrical failure or a human action, voluntary or not. The most likely cause for flap release could be moving the handle of the left pilot flap control switch to the 'down' position by an angle sufficient to trigger the switch. The design of the locking device of the ZPPM-45 switch of the left pilot and its geographical situation does not exclude the possibility of an unintentional flap release, which was revealed by a one-time check of these devices on 16 others Tupolev Tu-124 aircraft.

During a crop spraying flight in the Tambov region, the pilot-in-command got distracted while flying against the low sun when the aircraft lost height and crashed in a field. The aircraft was damaged beyond repair while both crew members were uninjured.
Probable cause:
Loss of control at low height after the pilot-in-command got distracted from operations.

Circumstances:
The crew left Morozovski at 0315LT on a supply mission to a site behind the German lines. On the return leg, a fire emerged in the cabin and spread rapidly. All 5 crew members bailed out at a height of some 350 meters (1 of them was killed and 1 seriously injured) while the sole passenger (an Air Force officer) did not manage to bail out and was killed. The aircraft crashed near Smelovka, about 5 km east of Verkhnyaya Tarasovka.

Circumstances:
The single engine airplane departed Tambov on a training mission, carrying two pilots, one cadet and one instructor. While cruising at an altitude of 1,000 metres in good weather conditions, the airplane started to disintegrate in the air, losing several pieces. It entered an uncontrolled descent and crashed in a field located in Alkakhatki, some 60 km southeast of Tambov. The aircraft was destroyed and both pilots were killed.
Probable cause:
It was determined that the student pilot probably made a mistake with the power lever while flying at a speed of 180 km/h. This caused the aircraft to reach the excessive speed of 260 km/h and to enter an overspeed. Several pieces from the wings, engine, ailerons and lower plant started to detach. The airplane then entered an uncontrolled descent and crashed. It was determined that the airplane was not certified to fly over the speed of 210 km/h.

Circumstances:
The single enginer airplane departed Tambov on a training flight to Balashov with one instructor and one student pilot on board. About 15-20 minutes into the flight, while flying over the village of Sukhotinka, the airplane was following a road at low height when it entered clouds. Shortly later, it rolled to the left, descended to the ground and crashed. Both crew members were killed.
Probable cause:
The accident was the consequence of the combination of the following factors:
- The student pilot was not authorized to fly in clouds,
- The student pilot training was insufficient,
- The instructor was overconfident.

Loss of control after the pilot suffered a loss of orientation (spatial disorientation) due to his inexperience.
