Circumstances:
The crew completed several spraying missions that day on behalf of the sovkhoz of Koptevsky. At the end of the afternoon, the crew took off and after departure, the pilot-in-command made a sharp turn to the left when the airplane banked left to an angle of 60° then stalled and crashed in flames in an open field located 4 km northwest of Koptevka. The copilot was killed while the captain was seriously injured. The aircraft was destroyed by a post crash fire. The captain died from his injuries seven days later.
Probable cause:
Hazardous manoeuvres after takeoff on part of the pilot-in-command who completed a turn over the tolerated 30° bank. Fatigue was considered as a contributing factor as well as the fact that the copilot was intoxicated with a level of 0,9‰.

While on a night supply mission to an area located behind the enemy lines, the airplane was shot down by a German fighter and crashed in Zhukovka. All 6 crew managed to bail out but one of them was killed and one was injured.

The crew was engaged in a supply mission to soldiers based behind the enemy lines in Belarus. En route, the airplane was shot down by a German fighter and crashed near Mogilev. The passenger and one crew member managed to bail out and survived while five other crew members were killed.

Circumstances:
In the evening of 17 July 1943, the airplane was dispatched to an airstrip near Osipovka with supplies for partisans. Because the ground was soaked, the crew was unable to takeoff and several attempts were abandoned as the airplane was unable to reach more than 50 km/h speed. The airplane was parked nearby a forest and camouflaged. Around 1930LT, the area was attacked by German fighters and several bullets hit the left engine, causing an oil leak. Following makeshift repairs, the crew took off at 0245LT. Around three to five seconds after liftoff, the left engine failed. The airplane rolled to the left, touched the ground and crashed nose first in a swampy area. All 15 occupants were injured, two of them seriously. About 45 minutes later, the airplane was destroyed by enemy fire.
Probable cause:
Failure of the left engine at takeoff.
