Circumstances:
Iranian ATR72 aircraft registered EP-ATS operated by Iran Aseman Airlines was assigned to perform a domestic scheduled passenger flight from Tehran to Yasuj on 07:55 local time. The aircraft took off from Tehran Mehrabad International Airport (0III) at 04:35 UTC. (08:05 LMT) and the flight was the first flight of the day for aircraft and the crew. The cruise flight was conducted at FL210 on airway W144 and no abnormal situation was reported by the crew and the flight was continued on Tehran ACC frequency till the time the first officer requested latest weather information of the destination by contact to Yasuj tower then requested to leave FL210 to FL170 from Tehran ACC. When the aircraft was descending to FL170 and crew calling YSJ tower the aircraft descending was continued to altitude of 15000 ft. The aircraft was approved to join overhead of the airport and perform “circling NDB approach “to land on RWY 31 at the destination aerodrome. Finally the aircraft collided with a peak lee of DENA Mountains about 8.5 miles at North far from the airport and involved accident on 06:01 UTC. The aircraft was completely destroyed as a result of collision with the mountain at the altitude of approximately 13,300 ft.
Probable cause:
The accident was happened due to many chains of considered causes but the “Human Factor” had main roll for the conclusion of the scenario. The Cockpit Crew action which has caused dangerous conditions for the flight is considered as main cause. Based on provided evidences, the errors of cockpit crew were as follows:
- Continuing to the Yasouj airport for landing against Operation manual of the Company, due to low altitude ceiling of the cloud and related cloud mass. They should divert to alternate airport,
- Descending to unauthorized altitude below minimum of the route and MSA,
- Lack of enough CRM during flight,
- Failure to complete the stall recovery (flap setting, max RPM),
- Inappropriate use of Autopilot after Stall condition,
- Inadequate anticipation for bad weather based on OM (Clouds, Turbulence, and Icing ...),
- Quick action to switch off anti-ice system and AOA,
- Failure to follow the Check lists and standard call out by both pilots.
Contributing Factors:
The contributive factors to this accident include but are not limited to the following:
- The airline was not capable to detect systematic defectives about :
- Effectiveness of crew training about Meteorology, OM, SOP,
- Enough operational supervision on pilot behaviors,
- The lack of SIGMET about Mountain Wave or Severe Mountain wave,
- Unclear procedure for stall recovery in FCOM,
- Lack of warning in aircraft manuals by manufacturer for flight crew awareness about mountain wave.
- Lack of APM System to alert crew about performance degradation.
Other Deficiencies and Short Comes:
In the process of the accident investigation, some detailed deficiencies and short comes were found and should be considered as latent conditions by related authorities:
- AD accomplishment and related monitoring,
- Sanction on aviation industries and effect on Flight safety,
- Non-standard communication between ATC and crew,
- Unclear definition of the Fully Qualified Pilot and qualified copilot in Aircrew regulation.
- Weather forecast (TAF) in the airports based on annex 3 procedure in the Civil Aviation Organization for approving alternative method of compliance for aircraft AD,s
- Search and rescue Coordination with local authorities for aviation accidents,
- Time setting of aircraft flight data recording(FDR) either by technician or pilots.

Circumstances:
The crew was performing a training flight on behalf of the Iranian constructor HESA (Iran Aircraft Manufacturing Industrial Company) which built the Antonov AN-140 under licence with the name HESA IrAn-140-100. On final approach to Isfahan-Shahin Shahr-HESA Airport, the aircraft crashed in unknown circumstances and was destroyed. All five occupants, one instructor and four pilots under training, were killed.

Circumstances:
The aircraft departed Kharkiv Airport on a charter flight to Isfahan, Iran, with an intermediate stop in Trabzon, carrying 38 passengers and six crew members. All the occupants were engineers working for Antonov and flying to Isfahan to conduct test flights on the first Iran-140, a prototype built under licence by HESA Company. While descending to Isfahan Airport by night and marginal weather conditions, the aircraft struck the slope of a mountain located 30 km from the airport. The aircraft was totally destroyed and all 44 occupants were killed.
Probable cause:
Controlled flight into terrain after the pilots failed to use properly the GPS system installed in the cockpit.

While descending to Tehran-Mehrabad Airport, the crew was informed about the poor weather conditions at destination with snow falls, low visibility and a 20 knots tailwind. The crew decided to divert to Isfahan-Shahid Beheshti Airport. On approach, the crew encountered limited visibility due to foggy conditions. The aircraft struck the ground, lost its undercarriage and slid for almost one km before coming to rest in a desert area located 8 km short of runway 26. All 113 occupants escaped uninjured while the aircraft was damaged beyond repair.

The aircraft was completing a flight from Kish Island to Tehran with an intermediate stop in Isfahan. Shortly after takeoff from Isfahan-Shahid Beheshti Airport, while climbing to the altitude of 2,000 feet, the crew reported pressurisation problems and was cleared to return for an emergency landing. Five minutes after its departure, the aircraft went out of control and crashed few km from the airport. All 12 occupants were killed, among them General Mansour Sattari, Chief of the Iranian Air Force.

Circumstances:
Thirty-five minutes after takeoff from Isfahan Airport, while cruising by night at the altitude of 18,000 feet, the aircraft entered an uncontrolled descent and crashed 47 seconds later on the slope of a mountain located in the Kharkas Mountain Range, near Natanz. The wreckage was found few hours later in an isolated area, near a telecommunication relay. The aircraft disintegrated on impact and all 66 occupants were killed.
Probable cause:
For unknown reasons, both engines lost power simultaneously, causing the aircraft to enter an uncontrolled descent.

On 16 April the aircraft departed Mehrabad Airport at 0228Z on a charter flight to Yazd and Bandar Abbas. On the same day the first stage of the return flight was made to Shiraz where an overnight stop was made. Next day EP-ABA left Shiraz at 0245Z to pick up four passengers at Marvdasht. At 0525Z it departed Marvdasht on a direct flight to Mehrabad Airport, Tehran, ETA 0800Z. A last contact was made with Esfahan at 0630Z reporting its position as 20 miles east of Esfahan - VFR, flight level 150, ETA Mehrabad 0800Z. This was the last message received from the aircraft. On 19 April word was received at Mehrabad Airport that the aircraft had crashed on the mountain 40 km south of Ardestan and 18 km west of Zafarkand Village. The pilot and four passengers were killed in the accident and the aircraft was destroyed.
Probable cause:
The pilot was unexpectedly confronted with severe carburetor icing conditions. A loss of engine power resulting from this impelled him to descend through the cloud layer when possibly the icing conditions became considerably worse, thus rendering a quick melting of the carburetor ice problematic. His already serious situation was intensified due to breaking cloud over mountainous terrain under conditions of bad visibility. In an effort to avoid the mountain barriers confronting him, he endeavored to fly through a narrow gorge and a visible gap at the top. He, however, to a still greatly diminished power from the persistent effects of carburetor icing, together with the most imposing turbulence and downdraught effects that he would undoubtedly encounter in the crest area, his attempt failed. Unfortunately, the protruding rock formation which he first hit presented a further great obstruction at the most critical point of his passage.
Final Report:
