Circumstances:
The crew departed Pevek on a special flight to Baranikha in good weather conditions with high clouds and a visibility of 20 km. While approaching Baranikha, weather conditions deteriorated and the visibility dropped to 5 km due to snow falls. While descending to an altitude of 330 meters, the airplane struck a snow covered hill, causing the left ski to be torn off. The airplane continued for dozen yards and eventually nosed down and came to rest in the snow. Both pilots were uninjured while the aircraft was damaged beyond repair.
Probable cause:
Investigations did no reveal any technical anomalies on the aircraft, the engine or any equipment. It is believed the pilot reduced too much the engine power on approach, causing the airplane to lose speed and height.

Circumstances:
During the takeoff roll, at a speed of 130 km/h, the pilot-in-command started the rotation. The airplane climbed few meters but was unable to gain sufficient height and banked right to an angle of 20°. The crew tried to regain control but the airplane was totally unstable. The pilot decided to reduce the power on left engine then on right engine when the airplane crash landed past the runway end. All six crew members were uninjured while the aircraft was damaged beyond repair.
Probable cause:
Poor flight preparation on part of the crew. Prior to departure, the airplane was refuelled but the fuel quantity was too high and the total weight of the aircraft at takeoff was 12,140 kilos, about 640 kilos above the MTOW. Also, it was determined that the cargo shifted in the cabin at liftoff and struck the lateral door, modifying the CofG by 22% and causing the aircraft to be uncontrollable.

The crew was completing a mail flight from Egvekinot to Anadyr. On final approach to Anadyr-Liman Airport, weather conditions were poor with a limited visibility due to clouds down to 30 meters from the ground. During the last segment, the pilot-in-command saw a man walking on the runway and initiated a right turn in strong cross winds. The single engine airplane stalled and crashed near the runway threshold. All six occupants were slightly injured while the aircraft was damaged beyond repair.
Probable cause:
It was determined that the man walking on the runway was dispatched to set fire on ground to guide the airplane on approach. Investigations reported that the crew has not been informed about the presence of the man walking on the runway and he has not been informed that an aircraft was scheduled to land.

On a flight from Nizhniye Kresty (now Cherski) to Pevek on a survey flight of the icy continent, the copilot decided to cut the route short when the captain was in the passenger cabin and overflew Cape Shelagski (469 meters). On approach to the cape, the aircraft was caught by strong downdraft and eventually struck the hill with the lower fuselage, was thrown up into the air again and eventually crashed into a large boulder and caught fire. Three crew members were rescued while seven others occupants were killed.
Probable cause:
Wrong decision on part of the copilot who decided to modify the route without referring to the captain and against all published procedures.

The crew was performing a short 10 minutes flight between both Anadyr Airports and the airplane was carrying two passengers, two crew and various goods for a total weight of 880 kilos. Shortly after takeoff, while climbing to a height of 40 meters, the aircraft stalled from the rear and crashed. All four occupants were injured and the aircraft was destroyed.
Probable cause:
It was determined that the crew failed to secure the load properly prior to takeoff. At rotation, the freight moved to the rear of the cabin, causing the CofG to be out of the enveloppe and the aircraft to be out of control.

The aircraft was carrying five scientists and three crew members who were involved in a geological mission of the Bolshaya Osinovaya River. Shortly after takeoff from Tanyurer, at an altitude of 80 meters, the pilot-in-command made a sharp turn to the left when the airplane stalled and crashed. All eight occupants were rescued and slightly injured while the airplane was destroyed.
Probable cause:
Poor piloting techniques on part of the captain who made too sharp turn to the left at low height (80 meters).

The aircraft departed Uelkal bound for Anadyr with seven passengers on board, a crew of five and 580 kg of various goods. While approaching Anadyr by night, the crew did not realized he was off course by eight km when, at an altitude of 720 meters, the airplane struck the slope of Mt Gora Ioanna (920 meters high) located about 35 km northeast of Anadyr. As the aircraft failed to arrive at destination, SAR operations were initiated and the burnt wreckage was found four days later on a 50° slope. The aircraft disintegrated on impact and all 12 occupants have been killed.
Probable cause:
Investigations were unable to determine the exact cause of the deviation. At the time of the accident, visibility was poor due to the night and clouds. The assumption that the aircraft was caught by downdraft was not ruled out.

About 15 minutes after takeoff from Anadyr, the aircraft hit the slope of a mountain located in the Zolotoy Mountain Range, northeast of Anadyr. While the occupant fate remains unknown, the aircraft was destroyed. It was reported by the Russian Authorities that the captain was known for his lack of discipline and his alcohol problems.

The crew was performing a flight from Kresty Kolymsky to Anadyr on behalf of the Chukotskaya Aviagruppa on this Tupolev G-1, a version of the ANT-4. The captain (M. N. Tomilin) decided to cut the route short when shortly later, he encountered below-minima weather conditions (poor visibility, snow and icing). Instead to return, he decided to continue in such conditions when the aircraft crash-landed in the foothills of the Pekulny range on the upper reaches of the Afonkin River, some 60 km east of Stalino (now Mukhomornaya). The aircraft was lost while all three crew members were slightly injured and rescued.
Probable cause:
Wrong decision on part of the captain to continue the flight in below-minima weather conditions.
