Circumstances:
The aircraft was subject to major maintenance during the last two months, especially with the rudder actuators that were replaced. As there were still problems with the actuators during the last flight, decision was taken to make the appropriate adjustments followed by a flight to Salekhard. During the takeoff roll on runway 16/34, at a speed of 150 km/h, the aircraft deviated to the left and the captain decided to counter this deviation by using the nosewheel steering system. This caused the right front tyre to burst. At a speed of 250 km/h, the takeoff procedure was abandoned but this decision was taken too late. Unable to stop within the remaining distance (the runway 16/34 is 2,530 metres long), the aircraft overran, lost its nose gear and rolled for 577 metres before coming to rest. All 11 occupants escaped uninjured while the aircraft was damaged beyond repair.
Probable cause:
The following findings were identified:
- Poor quality of work from the engineering personnel of Voronezh Avia during the replacement and adjustment of the hydraulic amplifier GU-108D,
- Unsatisfactory flying and technical operation of the aircraft after the completion of the replacement of the hydraulic amplifier, during which the incorrectly installed rudder actuator was not discovered,
- The decision taken by the crew to continue the takeoff procedure despite significant efforts needed for the deflection of the right rudder pedal already noted during the preflight checks,
- The late rejection of the takeoff procedure.

Circumstances:
The four engine aircraft departed Moscow-Vnukovo on a cargo flight to Sredny Island with an intermediate stop in Salekhard, carrying six crew members, seven passengers and a load of 9,6 tons of various goods for the personnel of the drifting Polar Station "Severny Polyus 32" (North Pole 32). On final approach to the Sredny Ostrov Airfield, the crew encountered poor weather conditions and limited visibility. Too low, the aircraft struck the icy ground about 650 metres short of runway. It lost its undercarriage and slid for few dozen metres before coming to rest 165 metres to the right of the extended centerline. All 13 occupants escaped uninjured while the aircraft was damaged beyond repair.
Probable cause:
The following findings were identified:
- Poor flight preparation,
- The crew did not receive a suitable weather briefing for the destination airport,
- Poor weather conditions at destination,
- The visibility was below minimums,
- The crew continued the approach without visual contact with the runway until the aircraft impacted ground,
- The crew failed to initiate a go-around procedure.

Circumstances:
The single engine airplane departed Salekhard Airport at 1903LT on a cargo flight to Mys Kamennyy, carrying a load of 41 alcohol bottles for a total weight of 800 kilos. During initial climb, while flying by a polar night at a speed of 230 km/h, the airplane went out of control and crashed in a snow covered tundra located about 8 km north of Salekhard Airport. The airplane was destroyed and both occupants were killed.
Probable cause:
The crew lost control of the airplane during a night flight after suffering a spatial disorientation caused by the failure of both AGK-47B artificial horizon and GPK-48 gyro compass. It was reported these instruments already failed before the accident without the cause having been established.

Circumstances:
The aircraft was performing a cargo flight from Salekhard to Tarko-Sale, carrying six crew members and a load of 11 tons of cement. Rotation was completed after a course of 1,250 meters. Immediately following takeoff, the airplane entered a steep nose-high attitude (angle of attack of 20°) then banked right and crashed one minute later in a field located 1,800 meters past the runway end. A crew member was rescued while five other occupants were killed.
Probable cause:
Failure of the crew to prepare the flight properly and to follow the pre-takeoff checklist. Takeoff maneuver was completed with the flaps still retracted, causing the airplane to stalled after rotation.

Circumstances:
The takeoff from Tazovskoye Airport was attempted from an icy runway and in marginal weather conditions with a visibility limited to 500 meters and a crosswind gusting to 43 km/h. After a takeoff roll of 1,600 meters, the pilot pulled up and started the rotation when the aircraft banked right and crashed 265 meters farther, about 70 meters on the right of the extended center line. All four crew members and eight passengers were killed while 13 passengers were injured. The aircraft was destroyed.
Probable cause:
The accident was the consequence of a loss of control while taking off in marginal weather conditions. The flight was dispatched despite the fact that the visibility and weather conditions were below minima at the time of the accident. It was also reported that ATC consumed alcohol the night before and failed to have sufficient rest time.

Circumstances:
The aircraft was performing a supply mission for the Ural expedition christened 'Zheldorproyekt'. On the leg from Salekhard to Abez, while cruising in poor weather conditions, the aircraft hit the slope of Mt Raiiz, in the Polyarnyy Ural Mountain Range. The wreckage was found few weeks later, in January 1949, and all 19 occupants were killed. The aircraft was owned by the Soviet Ministry of Internal Affairs (MVD – Ministerstvo Vnutrennikh Del).
