Circumstances:
En route from Barstow to Lake City, the pilot informed ATC about the failure of the right engine and was cleared to divert to Gainesville Airport. On final, in a limited visibility due to fog, the twin engine airplane collided with electronic towers and crashed in a field, bursting into flames. All three occupants were killed.
Probable cause:
Powerplant failure for undetermined reasons. The following contributing factors were reported:
- The pilot failed to see and avoid objects or obstructions,
- Inadequate maintenance and inspection of aircraft on part of the maintenance personnel,
- Fuel system: filters, strainers, screens - missing,
- Low ceiling,
- Fog,
- Complete failure of the right engine,
- Visibility two miles or less,
- Right fuel filter filtering element missing,
- Left filter sub amt of debris,
- Main oil screens abnormally dirty.

Circumstances:
Just after liftoff from Barstow-Daggett Airport, both engines n°3 and 4 lost power. The crew attempted an emergency landing when the airplane lost height and crashed in flames in an open field. A pilot was killed while five other occupants were injured. The aircraft was destroyed by a post crash fire. Illegal smuggling flight.
Probable cause:
Engine malfunction during initial climb due to inadequate preflight preparation. The following contributing factors were reported:
- Improper fuel grade,
- Improperly loaded aircraft, CofG,
- Failed to abort takeoff,
- Partial loss of power on two engines,
- Forced landing off airport on land,
- 2,981 lbs over maximum gross weight,
- N°3 & 4 engines showed severe detonation damages,
- Fueled with 100 octane,
- Smuggling flight.

Circumstances:
After takeoff from Davis Monthan AFB, the four engine airplane encountered difficulties to gain height. It struck power cables, stalled and crashed in flames. All three occupants were injured while the aircraft was destroyed.
Probable cause:
Inadequate preflight preparation on part of the crew. The following contributing factors were reported:
- Collision with wires,
- Lack of familiarity with aircraft,
- Failed to abort takeoff,
- Gust locks engaged,
- Operational supervisory personnel: inadequate supervision of flight crew,
- Unqualified person operated aircraft,
- Airport conditions,
- Checklist: failed to use,
- Downwind,
- Mechanic acting as copilot,
- 3,060 feet soft dirt strip.
