Circumstances:
The crew was completing a training flight from Kuala Lumpur-Subang Airport when on final approach to Butterworth Airbase, the twin engine aircraft went out of control and crashed, coming to rest against the perimeter fence. The aircraft was partially destroyed by impact forces and one crew member was killed while three other occupants were injured. Weather conditions were considered as good at the time of the accident.

Shortly after takeoff from RAF Butterworth, while climbing, one of the engine failed. The captain decided to attempt an emergency landing but the undercarriage collapsed on landing. The airplane slid for several yards before coming to rest. There were no casualties but the airplane was written off.
Probable cause:
Engine failure during takeoff.

Shortly after takeoff from RAF Butterworth, the left engine fire warning came on. The crew shot down the engine, feathered the propeller and obtained the permission to return for an emergency landing. On final, the airplane was too low, hit palm trees and crashed onto the runway. Four passengers were injured while 22 other occupants were unhurt. The aircraft was damaged beyond repair.
