Following a reconnaissance mission over Taormina, the crew was returning to Luqa Airport. On approach, the airplane was shot down by the pilot of a German fighter. The navigator was able to bail out before the airplane crashed one mile north of Dingli. Two other crew members remained on board and were killed.

While approaching the Marsaxlokk Bay at night, the float plane encountered downdraft from a height of 50 feet and crashed in the harbor, few yards offshore. All occupants were rescued, among them one was injured. The aircraft was damaged beyond repair. It was performing a stop in Valetta en route from Rome to Tripoli.

The crew was returning to RAF Hal Far following a local training sortie. For unknown reasons, the airplane landed hard. Upon impact, the undercarriage was torn off and the airplane went out of control. It veered off runway and came to rest, bursting into flames. Both crew members were injured but one died from his injuries four days later.
