Circumstances:
The pilot departed Everett-Payne Field in the evening of June 6 on a cargo service to Huron, SD. En route, he was informed about the presence of thunderstorms in the Huron area and decided to divert to Sioux Falls Airport where he landed at 0140LT. Awaiting weather improvement, he left Sioux Falls around 0415LT to resume his flight to Huron. Upon takeoff, the twin engine aircraft crashed in unknown circumstances and was destroyed. The pilot was killed.

Circumstances:
Shortly after a night takeoff from Huron-William Washington Howes Airport, while in initial climb, the seaplane stalled and crashed. Both occupants were seriously injured and the aircraft was damaged beyond repair. At the time of the accident, the OAT was 18° F (-8° C) with a relative high humidity.
Probable cause:
Inadequate preflight preparation on part of the pilot who decided to takeoff without deicing the airplane. The pilot took off with heavy accumulation of frost on wings.
