Circumstances:
The airplane departed Kubinka Airport near Moscow on a flight to Bol’shoye Veregovo. At 2100LT, it passed over the enemy lines then landed at Bol’shoye Veregovo Airfield at 2245LT. It took off at 2305LT and was shot down by enemy fire about 20 minutes later. It crashed in an open field and was totally destroyed. All 27 occupants were killed, among them wounded soldiers evacuated from the front.
Probable cause:
Shot down by German fire.

Circumstances:
The airplane departed Kuybyshev-Bezymyanka Airport on a special flight to Nizhny Taguil-Valyogin Bor Airport, carrying four crew members and two passengers, the director and his deputy of the n°18 Factory. While descending to the destination, the crew encountered marginal weather conditions and the visibility was poor. While descending to the altitude of 700 metres, the airplane impacted the slope of Mt Golaya (aka Mt Shaitan - 748 metres high) located in the Vesyolyye G Mountain Range. The wreckage was found two days later and all six occupants were killed. Manufactured last February, the airplane crashed before it could be entered into the register so it did not have any registration at the time of the accident.

Circumstances:
The airplane departed Vypolzovo on a cargo flight, carrying five crew members and a load of 2,4 tons of military equipment. After takeoff, the airplane climbed to a height of 50-70 metres then started to lose altitude. The left wing collided with a pole and was sheared off. Out of control, the airplane crashed on a wooded house located near the airfield. Four crew members were killed while a fifth occupant as well as two people in the house were injured.
Probable cause:
The accident was caused by the failure of the carburetor, causing a loss of power on the left engine and a subsequent loss of height.

Shortly after a night takeoff from Khvoynaya Airport, while climbing, the airplane went out of control and crashed. All six crew members were killed.
Probable cause:
It is believed that the accident was the consequence of an incorrect elevator trim tab and left ailerons setting prior to takeoff.

While taking off from the Gryadki Airstrip, the airplane collided with a second GVF PS-84 registered CCCP-L3497 and crashed. Two crew members were killed, a third was injured and three others escaped unhurt. The airplane was damaged beyond repair while the second was repaired.

The airplane departed Basra on a flight to Tehran on behalf of the Soviet Ministry of Defense. While cruising in poor visibility, the airplane impacted the slope of a mountain located in the Zagros Mountain Range. The wreckage was found about 32 km southeast of Darband. The aircraft disintegrated on impact and all 12 occupants were killed, among them Soviet officers, one USAAF Colonel and two employees of General Motors.
